Çocuk Cerrahisi Ameliyatlarında Ameliyat Sürelerinin Belirlenmesi

Öz

Objective: To know the duration of the operations increases efficiency and success in operation room organization. The aim of this study is to determine durations of 3 mostly performed operations in pediatric surgery. Material and Methods: Average preoperative, operative, and postoperative durations of total 860 cases in 3 main groups operated in the Department of Pediatric Surgery of Eskisehir Osmangazi University Medical Faculty, between 02.01.2007 and 03.08.2009 were determined. Circumcision, inguinal hernia and undescended testis were included in the study. Groups of inguinal hernia and undescended testis also had subgroups according to whether being unilateral or bilateral and having circumcision together. Results: Average age ranged from 1,6 to 5,7. When preoperative and postoperative waiting durations were analyzed, circumcision was the only operation that had a significant difference from others with a shorter duration comparing to the other subgroups. There were no significant differences between other subgroups. Average operation duration ranged from 16,9 minutes to 88,7 minutes for all subgroups. When operation durations were analyzed, it was found that average operation duration of circumcision and unilateral inguinal hernia was significantly shorter than the other subgroups. Conclusion: In this study, while determining the entry, operation and exit durations for 3 most common pediatric surgical operations; human, hospital environment, and patient factors related to the process were discussed. To know the duration of the surgical procedures provides a successful work flow in operation room and also may eliminate problems caused by disorganization

Çocuk Cerrahisi Ameliyatlarında Ameliyat Sürelerinin Belirlenmesi

Öz

Amaç: Hastane verimliliğinin artırılması ve başarılı bir ameliyathane organizasyonu için ameliyat sürelerinin bilinmesi önemlidir. Bu çalışma ile Çocuk Cerrahisinde en sık yapılan 3 cerrahi girişim için ameliyathane sürelerinin saptanması amaçlanmıştır. Gereç ve Yöntem: Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Çocuk Cerrahisi Anabilim Dalı’nda 02.01.2007–03.08.2009 yılları arasında 3 ana grupta ameliyatı gerçekleştirilen toplam 860 olguda ameliyethanede geçen ortalama ameliyat öncesi, ameliyat ve ameliyat sonrası süreleri belirlendi. Sünnet, kasık fıtığı, inmemiş testis nedeni ile opere olan olgular çalışmaya dahil edildi. Bu ana gruplar kasık fıtığı ve inmemiş testiste tek ve çift taraflı ve sünnetle beraber olmasına göre alt gruplara ayrıldı. Bulgular: Hastalık gruplarına göre ortalama yaş 1,6 yaş ile 5,7 yaş arasında değişmekteydi. Preoperatif ve postop bekleme süreleri değerlendirildiğinde sadece sünnetin diğer ameliyat alt grupları ile karşılaştırıldığında anlamlı farklılık gösterecek düzeyde kısa olduğu gözlemlendi. Diğer alt gruplar arasında anlamlı farklılık saptanmadı. Ortalama ameliyat süreleri tüm alt gruplar için 16,9 dakika ile 88,7 dakika arasında değişmekteydi. Ameliyat süreleri değerlendirildiğinde sünnet ve tek taraflı kasık fıtığı ortalama ameliyat sürelerinin diğer tüm altgruplar ile karşılaştırıldığında anlamlı olarak kısa olduğu saptandı. Sonuç: Bu çalışma, çocuk cerrahisinde en sık yapılan 3 ameliyat için ameliyathane giriş, ameliyat ve çıkış sürelerini tespit ederken, bu sürelere etki eden muhtemel insan ve ameliyathane ortamı ve hasta faktörlerini de tartışmıştır. Cerrahi girişimin sürelerinin bilinmesi ameliyathanede başarılı bir iş akışını sağladığı gibi yaşanabilecek olumsuzlukları da ortadan kaldırmaya yadımcı olur
