A REVIEW OF TRAUMATIC ASPHYXIA IN THE LITERATURE AND A CASE REPORT

Öz

Traumatıc Asphyxıa is commonly known as condition encountered in injuries caused by motor vehicles. It is thought that the closure of the glottis is secondary response to fear and that the intracavitary pressure increases and causes harmful situations, in this case where the physiopathology due to sudden thorax compression and closure of the glottis is important.

Our case, 18 year old man, no known diseases, suddenly work machine overturns and thorax was under pressure during 30 minutes. The glasgow coma scale (GKS) was observed as 7 in the emergency department and he was intubated. After that he was taken to intensive care and the diagnosis was made here. Treatment was started and the patient was discharged without any sequelae.

Traumatic asphyxia is rarely seen and can be mortal, can be easily treated with supportive treatment if there is no pathology except for isolated tirade.

TRAVMATİK ASFİKSİDE LİTERATÜRE YENİDEN BİR BAKIŞ VE BİR OLGU SUNUMU

Öz

Travmatik asfiksi genellikle motorlu araçlarla meydana gelen yaralanmalarda rastlanmış bir durum olarak bilinmektedir. Ani toraks kompresyonuna ve takiben glottisin kapanmasına bağlı gelişen fizyopatolojinin önemli olduğu olayda, glottisin kapanmasının korkuya sekonder bir cevap olduğu, göğüs içi basıncı arttırarak zararlı durumları ortaya çıkardığı düşünülmektedir.

Olgumuz 18 yaşında erkek, bilinen hiçbir hastalığı yok, üzerine aniden iş makinası devriliyor ve 30 dakika toraksın bası altında kaldığı söyleniyor. Acil serviste Glasgow koma skalası (GKS) 7 olarak gözlendi ve entübe edilip yoğun bakıma alındı. Tanısı yoğun bakımda netleştirilip tedavisi başlanan hasta herhangi bir sekel kalmadan taburcu edildi.

Nadir görülen ve mortal seyredebilen travmatik asfiksi, eğer izole triadının dışında bir patoloji yoksa destek tedavisiyle rahatça tedavi edilebilir.
