An Overview of Characteristics and Risk Factors of Peritoneal Dialysis-Related Peritonitis with One Center Experience

Objective: Differences in peritoneal dialysis (PD)-associated peritonitis rates worldwide may be due to differences in definitions as well as differences in practice and risk factors. We aimed to present our center data and to reveal our peritonitis characteristics and risk factors with literature recommendations. Materials and Methods: Total of 92 peritonitis episodes that developed in 51 (34,22%) of 149 patients receiving PD treatment between January 2009 - 2021 in our unit were retrospectively evaluated. During the follow-up period, peritonitis frequency, culture (-) peritonitis, peritonitis-free-patient, hospitalization due to peritonitis, transition to permanent hemodialysis (HD), peritonitis occurring after PD catheter insertion and prior to commencement of PD treatment (Pre-PD peritonitis), mortality rates were calculated on annual basis and peritoneal fluid culture growths were evaluated. In addition, peritonitis risk factors were analyzed in all patients who received PD treatment during this period. Results: In our center over the years, the frequency of peritonitis was maximum (max) 0.38 episodes/patient year, culture (-) peritonitis max: 75%, peritonitis-free patients max: 98%, hospitalization due to peritonitis max: 75%, permanent HD transition max: 50%, Pre-PD peritonitis max: 9%, no mortality due to peritonitis was observed. Gram (Gr) + bacteria were found in 68.4% (n=39) of peritoneal fluid cultures. Daily urine volume (p=0.02), serum albumin (p=0.02), potassium (p=0.02) and hemoglobin levels (p=0.01) were lower in the group with peritonitis compared to the group with out peritonitis. Conclusion: Regular evaluation of the peritonitis characteristics of each center determines the need to revise the practices of the centers. Additionally, awareness of peritonitis risk factors will contribute to the improvement of peritonitis rates by enabling the implementation of necessary interventions in this regard.
