THE USE OF DOPPLER PERFUSION INDEX IN THE DETECTION OF COLORECTAL CANCER HEPATIC METASTASES

Öz

ABSTRACT
Objective
Colon cancer, which is the most common type of cancer in the gastrointestinal system, is a major cause of morbidity and mortality. It usually spreads over the liver. During the detection of the diseases, the existence of metastases in the liver cannot be overlooked. Additional liver metastases can also be observed in the monitoring period. Early diagnosis of liver metastases in colon cancer bears utmost importance. The use of perfusion changes in liver during the early diagnosis has been a popular research topic. Doppler Perfusion Index can be defined as a term which occurs as a result of increasing hepatic arterial blood flow or decreasing portal venous blood flow and denotes the ratio of hepatic arterial blood flow to total liver blood flow. In this respect, the present study aims to detect the existence of micrometastasis using DPI in patients suffering from colon cancer and displaying no signs of visible liver metastasis.
Material and method
We measured DPI index in 79 patients suffering from colon cancer and displaying no signs of visible liver metastasis and monitored them in terms of prospectively visible metastasis development.
Findings
During the monitoring process, 2 patients developed exitus and 9 patients developed metastasis. Thus, it can be stated that DPI values obtained from our patient group overlapped with the findings in the existing literature in terms of the early diagnosis of metastasis.
Conclusion
Offering an inexpensive and non-invasive method for direct measurement, Duplex RDUS method can be used in the early diagnosis of hepatic metastasis in patients with colorectal cancer.

Kolorektal Kanser Hepatik Metastazlarının Saptanmasında Doppler Perfüzyon İndeksi

Öz

Amaç
Kolon kanseri gastrointestinal sistemin en sık görülen kanseri olup önemli bir morbidite ve mortalite sebebidir. En sık yayılım gösterdiği organ karaciğerdir. Hastalığın tanısı anında karaciğerde metastaz varlığı azımsanamayacak derecededir. Takip sürecinde de ayrıca karaciğer metastazları gelişmektedir. Kolon kanserinin karaciğer metastazlarının erken tanısı önemlidir. Erken tanıda karaciğer perfüzyon değişikliklerinin kullanılabileceği ötedenberi çalışma konusu olmuştur. Hepatik arteryel kan akımında artış yada portal venöz kan akımında azalma ile ortaya çıkan ve hepatik arteryel kan akımının total karaciğer kan akımına oranını ifade eden Doppler Perfüzyon İndeksi olarak tanımlanmış bir kavram bulunmaktadır. Biz de karaciğerde görünür metastazı bulunmayan kolon kanserli hastalarda DPİ ile mikrometastaz varlığını tesbit etmeyi amaçladık.
Gereç ve yöntemler
Kolon kanseri tanılı ve karaciğerinde görünür metastazı bulunmayan 79 hastanın DPİ ölçümlerini yapıp prospektif olarak görünür metastaz gelişimi yönünden takip ettik.
Bulgular
Takibimiz esnasında 2 hastada exitus ve 9 hastada görünür metastaz gelişmiş olup hasta grubumuzda elde ettiğimiz DPİ değerleri metastaz varlığını erken tesbit etmede literatürle uyumlu olarak değerlendirilmiştir.
Sonuç
Ucuz, noninvazif ve doğrudan ölçüm yapılabilen dupleks RDUS yöntemiyle, kolorektal kanserli hastaların hepatik metastazlarının erken aşamada tesbit edilebileceğini düşünüyoruz.
