The Role of Colposcopic Biopsy Number in the Diagnosis of Cervical Premalignant Lesions

Öz

Objective: Currently, there is no accepted standard number of biopsies for colposcopy-guided cervical biopsy. While the most common practice is a single biopsy from a suspicious area, some centers implement protocols that involve multiple biopsies. In our study, we aimed to investigate the effect of multiple cervical biopsies accompanied by colposcopy on the detection of high-grade lesions of the cervix.

Materials and Methods: The files of 170 patients who underwent colposcopy at xxx Hospital between January 1, 2020, and December 31, 2024, were retrospectively reviewed. Cervical cytology results (ASCUS, AGC, LSIL, HSIL, ASC-H), HPV test results, and demographic information were recorded.

Results: Among patients who underwent colposcopy-guided cervical biopsy, 69 had CIN 1 lesions (40.6%), 62 had normal appearance (36.5%), and 39 had CIN 2 + CIN 3 lesions (22.9%). CIN 2 + CIN 3 lesions were observed to be detected more frequently than CIN 1 lesions in patients who underwent 3 or 4 cervical biopsies (p<0.005).

Conclusion: Increasing the number of cervical biopsies during colposcopy can be considered a significant strategy, particularly in detecting high-grade lesions.

Serviks Premalign Lezyon Tanısında Kolposkopik Biyopsi Sayısının Rolü

Öz

ÖZET

Amaç: Günümüzde kolposkopi eşliğinde yapılan serviks biyopsi için kabul edilen standart bir biyopsi
sayısı bulunmamaktadır. En yaygın uygulama şüpheli alandan tek biyopsi olsa da bazı merkezlerde
birden fazla biyopsi alınmasını içeren protokoller uygulamaktadır. Çalışmamızda kolposkopi eşliğinde
çoklu servikal biyopsi alınmasının serviksin yüksek dereceli lezyonlarının tespitindeki etkisini
incelemeyi amaçladık.

Materyal ve Metod: xxx Hastanesinde 01 Ocak 2020 ve 31 Aralık 2024 tarihleri arasında kolposkopi
işlemi yapılan toplam 170 hastanın dosyaları retrospektif olarak tarandı. Servikal sitoloji sonuçları
(ASCUS, AGC, LSIL, HSIL, ASC-H), HPV test sonuçları ve demografik bilgileri kaydedildi.

Bulgular: Kolposkopi eşliğinde serviks biyopsi alınan 170 hastanın 69’unda CIN 1 (%40,6), 62’sinde
normal patoloji (%36,5) ve 39’unda CIN 2 + CIN 3 (%22,9) lezyon saptanmıştır. Serviksten 3 veya 4
adet biyopsi alınan hastalarda CIN 1 lezyonuna göre CIN 2 + CIN 3 lezyonlarının daha sık saptandığı
gözlemlenmiştir (p<.005).

Sonuç: Kolposkopide serviks biyopsi sayısını arttırmak özellikle yüksek dereceli lezyonların tanısının
saptanmasında anlamlı katkı sağlayan bir strateji olarak değerlendirilebilir.
