Use of Extracorporeal Membrane Oxygenation in COVID-19

Öz

Coronaviruses are viruses that can cause disease in humans and animals. Generally, the disease involves the upper and lower respiratory tract. In late 2019, atypical pneumonia cases occurred in Wuhan, China. The most common symptoms in cases have been reported to be fever, cough and respiratory distress. Later, this coronavirus was named Covid-19 by the World Health Organization and was declared a pandemic. Acute Respiratory Distress Syndrome (ARDS) caused by Covid-19 is of great importance. Conventional treatment of ARDS is usually managed by adjuvant neuromuscular blockade and mechanical ventilation, depending on the severity of hypoxemia. Extracorporeal membrane oxygenation (ECMO) is a cardiopulmonary support tool for respiratory and heart failure. ECMO can be used in cases where conventional treatment has failed. Developments in ECMO technology and the reliability of ARDS in previous treatments increase its importance in Covid-19. It should be noted, however, that the role and outcome of ECMO in Covid-19 therapy is currently uncertain, requiring more clinical trials. As a result, ECMO can be used in Covid-19 therapy, ARDS and other indicative conditions where conventional therapy has failed.

COVID-19'da Ekstrakorporeal Membran Oksijenasyonun Kullanımı

Öz

Koronavirüsler inanlarda ve hayvanlarda hastalık yapabilen virüslerdir. Genellikle hastalık üst ve alt solunum yollarını tutmaktadır. 2019 sonlarında Çin'nin Wuhan kentinde atipik pnömoni vakaları görülmüştür. Vakalarda en sık görülen belirtilerin ateş, öksürük ve solunum sıkıntısı olduğu bildirilmiştir. Daha sonra bu koronavirüs Dünya Sağlık Örgütü tarafından Covid-19 olarak adlandırılmış ve pandemi ilan edilmiştir. Covid-19'un neden olduğu Akut Solunum Sıkıntısı Sendromu (ARDS)'da büyük bir önem taşımaktadır. ARDS'nin konvansiyonel tedavisi genellikle hipokseminin şiddetine göre adjuvan nöromüsküler blokaj ve mekanik ventilasyon ile yönetilir. Ekstrakorporeal membran oksijenasyonu (ECMO), solunum ve kalp yetmezliği için bir kardiyopulmoner destek aracıdır. ECMO, konvansiyonel tedavinin başarısız olduğu durumlarda kullanılabilir. ECMO teknolojisindeki gelişmeler ve ARDS'nin geçmiş dönem tedavilerindeki güvenirliği Covid-19'da da önemini artırmaktadır. Ancak ECMO'nun Covid-19 tedavisindeki rolü ve sonucunun şu anda belirsiz olduğunu, daha çok klinik çalışma gerektirdiğini belirtmek gerekir. Sonuç olarak Covid-19 tedavisinde ECMO, konvansiyonel tedavinin başarısız olduğu ARDS ve diğer endikasyonel durumlarda kullanılabilir.
