Mean Platelet Volume (MPV) for Predicting Prognosis of Rectum Cancer After Neoadjuvant Treatment

Öz

Background:
Rectum cancer is a
type of colorectal cancer. Its etiology and etiopathogenesis are similar to
other colon diseases. However, it is differentiated from other colon tumors
because of its different anatomy and treatment. Although there are previous
studies indicating high mean platelet volume (MPV) levels with poor prognosis
in colorectal cancers, there is no study evaluating its prognostic value in
rectum cancer.Based on this fact, we aimed to evaluate the MPV value for
predicting prognosis of rectum cancer patients who treated with neoadjuvant
chemotherapy.

         Method: We retrospectively
collected the data of 80 operated rectum adenocarcinoma patients who were
treated neoadjuvant chemoradiotherapy between 2011 and 2018 in Umraniye
Research and Training Hospital and Acıbadem University Medical Oncology
Outpatient Clinic.The clinical parameters were recorded at the time of
diagnosis such as; histopathological type of cancer (adenocarcinoma or
mucinous), histological differentiation, stage, relapse status and
localization, presence of metastasis, total lymph node numbers.Peripheral
venous blood samples were collected for analysis of of MPV, lymphocyte (Lym),
neutrophile (Neu) and platelet (Plt) numbers,, hemoglobin levels and
carcinoembryonic antigen (CEA) levels at the time of diagnosis. MPV
was investigated as prognostic factors for disease
free survival

        Findings: This study
included 80 rectum cancer patients and 55 were male (68,8%). Median age was 56
(range 22 to 83 years). According to clinical parameters, histopathological
types were adenocarcinoma in 75 (93,8%) patients and mucinous type in 5 (6,2%)
patients
). In multivariate
analysis, MPV were  found to be
independent prognostic factors for disease free survival (p 0.02).









 Interpretation: According
to our study, we suggest that high levels of MPV at the time of diagnosis can
be used as a predictive biomarker for rectum patients who may show relapse
predisposition

Rektum Kanserinin Prognozunu Öngermede Ortalama Trombosit Hacmi (MPV)'nin Rolü

Öz

Amaç:
Rektum kanseri, bir kolorektal kanser türüdür.
Etiyolojisi ve etiyopatogenezi diğer kolon hastalıklarına benzer. Bununla
birlikte, farklı anatomisi ve tedavisi nedeniyle diğer kolon tümörlerinden
ayrılır. Kolorektal kanserlerde prognozu kötü olan yüksek ortalama trombosit
volümü (MPV) düzeylerini gösteren önceki çalışmalar olmasına rağmen, rektum
kanserinde prognostik değerini değerlendiren bir çalışma yoktur. Bu nedenle
neoadjuvan tedavi alan rezeke rektum kanseri hastalarının prognozunu tahmin
etmek için MPV değerini rolünü değerlendirmeyi amaçladık. 



Yöntem:
Ümraniye Eğitim ve Araştırma Hastanesi ve Acıbadem
Üniversitesi Medikal Onkoloji Polikliniğinde 2011-2010 yılları arasında
neoadjuvan kemoradyoterapi alan 80 ameliyatlı rektum adenokarsinomlu hastanın
verilerini retrospektif olarak topladık. Klinik parametreler; histopatolojik
tip kanser (adenokarsinom veya müsin), histolojik farklılaşma, evre, nüks
durumu ve lokalizasyonu, metastaz varlığı, total lenf nodu sayıları. MPV,
lenfosit (Lym), nötrofilin (Neu) analizi için periferik venöz kan örnekleri
alındı. ve tanı sırasında trombosit (Plt) sayıları, hemoglobin seviyeleri ve
karsinoembriyonik antijen (CEA) seviyeleri. MPV, hastalıksız sağkalım için
prognostik faktörler olarak araştırıldı.



Bulgular: Bu çalışmaya 80 rektum kanseri hastası dahil edildi
ve 55'i erkekti (% 68,8). Ortanca yaş 56 (dağılım 22-83) idi. Klinik
parametrelere göre histopatolojik tipler 75 (% 93,8) hastada adenokarsinom, 5
(% 6,2) hastada müsin tip idi. Çok değişkenli analizde MPV'nin hastalıksız
sağkalım için bağımsız prognostik faktörler olduğu bulundu (p 0.02).



Sonuç: Çalışmamıza göre, tanı sırasındaki yüksek MPV
seviyelerinin nüks yatkınlığı gösterebilecek rektum hastaları için prediktif
bir biyobelirteç olarak kullanılabileceğini öneriyoruz.
