Sessiz ama Önemli: 15 Olguluk Seride Perikardiyal Kistlerin Cerrahi Tedavi Deneyimi

Perikardiyal kistler, mediastenin nadir görülen benign lezyonlarıdır ve çoğu zaman asemptomatik olarak seyreder. Bu çalışmada, kliniğimizde cerrahi olarak tedavi edilen perikardiyal kist olgularını retrospektif olarak analiz ederek literatürle karşılaştırmak amaçlandı. Temmuz 2012 ile Ağustos 2021 tarihleri arasında cerrahi olarak tedavi edilen 15 erişkin hasta çalışmaya dahil edildi. Klinik, radyolojik ve cerrahi veriler retrospektif olarak incelendi. Tüm hastalara toraks BT ve akciğer grafisi uygulandı. Bazı olgularda PET-BT ve EKO gibi ileri görüntüleme yöntemleri kullanıldı. Tüm kistler cerrahi olarak çıkarıldı ve histopatolojik olarak değerlendirildi. Hastaların %66,7’si asemptomatikti ve olguların %73.3’ünde kist sağ kardiyofrenik açıda yerleşmişti. En sık tercih edilen cerrahi yöntem video yardımlı torakoskopik cerrahi (VATS) idi. Ortalama kist çapı 4.43±2,34 cm, drenaj süresi 1.6±0.72 gün ve hastanede yatış süresi 2.73±1.09 gün idi. İki hastada (%13,3) postoperatif komplikasyon gelişti. Hiçbir olguda nüks izlenmedi. Perikardiyal kistler genellikle iyi huylu olmakla birlikte ciddi komplikasyonlara yol açabilir. Cerrahi eksizyon, düşük nüks oranı ve minimal invaziv yöntemlerin sağladığı avantajlarla etkili ve güvenli bir tedavi seçeneğidir.

Silent But Significant: Surgical Treatment Experience of Pericardial Cysts in a Series of 15 Cases

Pericardial cysts are rare benign lesions of the mediastinum and are often asymptomatic. This study aims to retrospectively analyze pericardial cyst cases surgically treated in our clinic and compare the findings with the literature.Fifteen adult patients who underwent surgical treatment between July 2012 and August 2021 were included in the study. Clinical, radiological, and surgical data were retrospectively reviewed. All patients underwent chest CT and radiography. Advanced imaging such as PET-CT and echocardiography was performed in selected cases. All cysts were surgically removed and examined histopathologically.A total of 66.7% of patients were asymptomatic, and 73.3% of cysts were located in the right cardiophrenic angle. The most commonly used surgical approach was video-assisted thoracoscopic surgery (VATS). The mean cyst diameter was 4.43±2.34 cm, the average drainage duration was 1.6±0.72 days, and the mean hospital stay was 2.73±1.09 days. Postoperative complications occurred in two patients (13.3%). No recurrences were observed.Although pericardial cysts are generally benign, they may lead to serious complications. Surgical excision is a safe and effective treatment, especially when performed using minimally invasive techniques due to their low recurrence rates and faster recovery.
