Protective Effects of Curcumin on One-Lung Ventilation–Induced Lung Injury in a Rat Model

Öz

Background: One-lung ventilation (OLV), routinely employed in thoracic surgery, induces regional ischemia followed by reperfusion during re-expansion of the collapsed lung. This sequence provokes oxidative stress, inflammatory activation, vascular leakage, and tissue injury. Curcumin, a polyphenolic compound with strong antioxidant and anti-inflammatory properties, has demonstrated protective effects in ischemia–reperfusion (I/R) models, yet its role in OLV-related lung injury remains insufficiently clarified. Objective: To determine whether curcumin attenuates biochemical and histopathological markers of pulmonary I/R injury in a rat OLV model. Methods: Thirty male Sprague–Dawley rats were randomized into Control, DMSO, and Curcumin groups (n=10 each). All underwent 60 minutes of right-lung ischemia via left-lung OLV, followed by 30 minutes of reperfusion with two-lung ventilation (TLV). Lung tissue samples were collected after OLV and TLV. Biochemical markers included malondialdehyde (MDA), superoxide dismutase (SOD), and tumor necrosis factor-α (TNF-α). Histopathological injury was scored for congestion, edema, hemorrhage, and inflammatory cell infiltration. Results: Biochemical parameters showed no significant differences among groups (p>0.05). In contrast, histopathological injury was markedly higher in Controls compared with Curcumin and DMSO (p<0.0001). Curcumin significantly reduced congestion, edema, hemorrhage, and inflammatory infiltration at both ischemia and reperfusion phases. Protective effects were comparable to DMSO, with both superior to Control. Conclusion: Curcumin mitigates structural lung injury associated with OLV-induced ischemia and reperfusion, despite limited systemic biochemical changes. These findings support curcumin as a promising adjunct for perioperative lung protection. Further studies with optimized dosing, improved delivery, and extended reperfusion evaluation are warranted.
