Lower Extremity Overuse Injuries in Children: A Single-Center Observational Study

With increasing sports participation among children, lower-extremity overuse injuries are being encountered more frequently. Single-center clinical data from Türkiye evaluating their relationship with sport type, sex, and involvement patterns remain limited. This study aimed to examine the clinical distribution of lower-extremity overuse injuries in a single-center pediatric orthopaedic cohort in Ankara. The records of children who presented with lower-extremity pain between September 2022 and February 2026 and were diagnosed with Sever’s disease, Osgood-Schlatter disease, Sinding-Larsen-Johansson disease, Iselin disease, or stress fracture were retrospectively reviewed. Regular sports participation was defined as participation in the same sport or physical activity for at least three days per week and at least one hour per day during the previous six months. Frequencies were compared using the chi-square test.
Sixty-nine children were included (52 boys, 17 girls; mean age 11.8 years, range 8–15). Regular sports/activity participation was present in 54 cases (78.3%), and unilateral involvement was observed in 55 cases (79.7%). The most common diagnoses were Osgood-Schlatter disease and Sever’s disease. Iselin disease was the only diagnosis more frequent in girls (P=0.0012). Basketball was the most frequently reported sport, followed by football. Basketball- and football-related injuries occurred only in boys, whereas volleyball-related injuries occurred only in girls. All patients were treated conservatively. Pediatric lower-extremity overuse injuries were closely associated with regular running- and jumping-based activities. Diagnostic distribution, sport type, and sex-related participation patterns may vary according to geographical and cultural characteristics.
