Ülseratif kolitle ilişkili dirençli pyoderma gangrenosum tedavisinde certolizumab pegol deneyimi

Öz

Öz: Pyoderma
gangrenosum tekrarlayan kutanöz ülserlerle karakterize nötrofilik bir
dermatozdur. Hastaların yarısında inflamatuvar barsak hastalığı vardır.
Tedavisinde temel ilaç kortikosteroidlerdir. Ancak steroidden kurtarıcı tedavi
olarak immunsupresif ilaçların kullanılması da gerekmektedir. Dirençli pyoderma
gangrenosum tedavisi hastayı ve hekimi zorlayabilmektedir. Dirençli vakalarda
biyolojik ilaçlardan infliximab başta olmak üzere anti-TNF ajanlarınn etkili olduğunu
gösteren olgu bildirileri vardır. Pegile monoklonal anti-TNF ajan olan
certolizumab pegolun etkisini gösteren literatürde sadece 2 olgu vardır. Bu
yazıda ülseratif kolit ile ilişkili dirençli pyoderma gangrenosumda
certolizumab pegolun etkinliğini gösteren literatürdeki ilk vaka sunularak
dirençli pyoderma gnagrenosumda alternatif tedavi seçeneği olarak certolizumab
pegole dikkat çekilmeye çalışılmıştır.

Certolizumab Pegol Experience in the Treatment of Ulcerative Colitis Resistant Pyoderma Gangrenosum

Öz

Abstract: Pyoderma gangreosum is a neutrophilic dermatosis
characterized with reiterative cutaneus ulcers. Underlying inflammatory bowel
disease is determined at half of the patients. The main drug in treatment is
corticosteroids. However, immunosuppressive drugs should also be used as
steroid-sparing treatments. Theraphy of resistant pyoderma gangrenosum  can challenge patients and physicians. In
resistant cases, there are case reports showing that anti-TNF agents,
especially infliximab, are effective. There are only 2 cases in the literature
demostrating the effect of certolizumab pegol, a pegylated monoclonal anti-TNF
agent. In this article, the first case about efficacy of certolizumab pegol in
a resistant pyoderma gangrenosum case 
associated with ulcerative colitis is presented, and it has been tried
to call attention to certolizumab pegole as an alternative treatment option in
resistant pyoderma gangrenosum.
