Hemodiyaliz Tedavisi Uygulanan Hastalarda Ksilitollü Sakızın Susama, Ağız Kuruluğu ve Bulantıya Etkisinin Değerlendirilmesi
Öz
Amaç: Çalışmanın amacı hemodiyaliz tedavisi alan hastalarda sakız çiğnemenin susama, ağız kuruluğu ve bulantıya etkisini değerlendirmektir.
Yöntem: Yarı deneysel tipteki bu çalışma, Rize’de bir hemodiyaliz merkezinde 75 hasta ile yürütülmüştür. Hastalar müdahale, kontrol I ve kontrol II olmak üzere üç gruba ayrılmıştır. Müdahale grubuna, altı hafta süresince günde beş kez 10 dakika ksilitollü sakız çiğnetilmiştir. Kontrol I grubu günde üç kez iki püskürtme şeklinde ağız spreyi kullanmış, kontrol II grubuna ise bir uygulama yapılmamıştır. Veriler; Hasta Bilgi Formu, VAS, Rhodes Bulantı-Kusma-Öğürme İndeksi ile toplanmıştır. Hastaların tükürük akış hızı dereceli tüp ile, ağız ortamı pH’sı pH metre ile ölçülmüştür. Veriler SPSS 22.0 programı ile değerlendirilmiştir.
Bulgular: Sakız kullanan hastaların susama ve ağız kuruluğu 2., 4. ve 6. haftalarda azalırken, kontrol I grubu hastalarında susama ve ağız kuruluğu yalnızca 4. haftada azalmış 6. haftada değişim görülmemiştir. Sakız kullanan hastalarda, “bulantı deneyimi” ve “bulantı sıkıntısı” ilk karşılaşmaya kıyasla 2., 4. ve 6. haftalarda anlamlı olarak azalmıştır (p<0.01). Kontrol I ve Kontrol II gruplarında “bulantı deneyimi”, “bulantı oluşumu” ve “bulantı sıkıntısı” alt boyutlarında istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05).
Sonuç: Bu çalışma, hemodiyaliz tedavisi uygulanan hastalarda ksilitollü sakız çiğnemenin susama, ağız kuruluğu ve bulantının azaltılmasında etkili olduğunu göstermektedir. Hemodiyaliz tedavisi uygulanan hastalarda ksilitollü sakız, hemşirelerin bu semptomları hafifletmek ya da azaltmak için kullanabileceği bir yöntem olarak önerilebilir.

Evaluation of the Effect of Xylitol Gum on Thirst, Dry Mouth and Nausea in Patients undergoing Hemodialysis
Öz
Objective: This study aims to evaluate the effects of chewing gum on thirst, dry mouth, and nausea in patients undergoing hemodialysis.
Methods: This quasi-experimental study was conducted with 75 patients at a hemodialysis center in Rize. Patients were divided into three groups: intervention, control I, and control II. The intervention group chewed xylitol gum for 10 minutes, five times a day for six weeks. Control I group used mouth spray three times a day with two sprays per use, while no intervention was applied to Control II. Data were collected using the Patient Information Form, VAS, and Rhodes Index of Nausea, Vomiting, and Retching. Patients' salivary flow rate was measured with a graduated tube, and oral pH was measured with a pH meter. SPSS 22.0 program was used for analysis.
Results: In patients using gum, thirst and dry mouth decreased at 2nd, 4th, and 6th weeks, whereas in the control group I, a reduction was observed only at 4th week, with no change at 6th week. In patients using chewing gum, “nausea experience” and “nausea distress” decreased significantly in 2nd, 4th and 6th weeks compared to the first encounter (p<0.001). No statistically significant difference was found in the sub-dimensions of “nausea experience”, “nausea occurrence” and “nausea distress” in Control I and Control II groups (p>0.05).
Conclusions: This study shows that chewing xylitol gum reduces thirst, dry mouth, and nausea in patients undergoing hemodialysis. Xylitol gum could be recommended as a method for nurses to alleviate or reduce symptoms in patients undergoing hemodialysis.
