Autoimmune hepatitis case suspected of toxic hepatitis and diagnosed after treatment

Abstract

Autoimmune hepatitis (OIH) is a chronic, necroinflammatory liver parenchymal disease that occurs in the absence of an etiologic agent known to cause chronic hepatitis in the liver. OIH presents with various clinical variants. Some patients with cholestatic liver disease (such as primary biliary cholangitis [PBC] and primary sclerosing cholangitis [PSC]) have clinical and serological features that indicate the presence of autoimmune hepatitis and may respond to immunosuppressive therapy. However, sometimes the diagnosis cannot be clarified in patients and it may be necessary to go from treatment to diagnosis. In this article, we presented a 33-year-old autoimmune hepatitis-cholangitis case, who was admitted with elevated aminotransferase and bilirubin levels, pruritus and icterus, the diagnosis of AIH could not be clarified after evaluation and was thought to be toxic hepatitis at first, but diagnosed after treatment. In conclusion, autoimmune hepatitis-cholangitis should be kept in mind in the differential diagnosis of all cases with elevated liver enzymes.

Toksik hepatit düşünülen, tedaviden tanıya gidilen otoimmun hepatit vakası

Öz

Otoimmun hepatit (OIH) karaciğerde kronik hepatite yol açtığı bilinen bir etyolojik ajanın yokluğunda ortaya çıkan, kronik, nekroinflamatuar bir karaciğer parankim hastalığıdır. OIH çeşitli klinik varyantlarla karşımıza çıkmaktadır. Bazı kolestatik karaciğer hastalığı olan hastalar (primer biliyer kolanjit [PBC] ve primer sklerozan kolanjit [PSC] gibi), otoimmün hepatit varlığını gösteren klinik ve serolojik özelliklere sahiptir ve immünosüpresif tedaviye cevap verebilirler. Ancak bazen hastalarda tanı netleştirilemez ve tedaviden tanıya gitmek gerekebilir. Bu yazıda 33 yaşında aminotransferaz ve bilirubin yüksekliği, kaşıntı ve ikter nedeniyle başvuran, değerlendirme sonrası OİH tanısı netleştirilemeyen ve öncelikle toksik hepatit olabileceği düşünülen ancak takipleri sonrası tedaviden tanıya ulaştığımız otoimmun hepatit-kolanjit vakamızı sunduk. Sonuç olarak karaciğer enzim yüksekliği olan tüm vakalarda ayırıcı tanıda otoimmun hepatit-kolanjit akılda tutulmalıdır.
