An unusual case report: retroperitoneal hydropic leiomyoma

Leiomyomas are one of the most prevalent gynecological conditions that cause bleeding in women, particularly in the postmenopausal and perimenopausal stages. Nonetheless, one of the rarest forms of myoma is retroperitoneal hydropic myoma. A 45-year-old woman with a history of four pregnancies, two cesarean sections, one vaginal delivery, and one abortion will be the subject of this case presentation. During her preoperative examination, a retroperitoneal hydropic myoma was discovered to be an adnexal mass. The patient's preoperative pelvic MRI was interpreted as follows: Approximately 15 x 16 cm in size, a giant cystic mass lesion filled the pelvis completely and extended superiorly to the umbilical level. It contained contrast-enhancing septal components, and in the superior left half, there was a contrast-enhancing and diffusion-restricting solid component that measured about 4 x 3 cm and was believed to be from the right ovary (mucinous ovarian carcinoma?). The uterus is being forced anteriorly by the giant mass lesion that has been described. A retroperitoneal cystic tumor was removed during the procedure. A hydropic leiomyoma without necrosis or hemorrhage was discovered by microscopic and histopathologic analysis. The hydropic degeneration of the leiomyoma and its uncommon location make this case remarkable. Furthermore, it was difficult to make an accurate diagnosis because it initially resembled an ovarian cyst.
