Üçüncü Basamak Bir Sağlık Kuruluşunda Son 3 Yılda İzlenen Hipoksik İskemik Ensefalopatili Olgular ve Kısa Dönem Klinik Seyirleri

Öz

Giriş: Hipoksik iskemik ensefalopati, yenidoğan yoğun bakım uygulamalarındaki tüm gelişmelere rağmen neonatal ölüm, serebral palsi ve ileri dönemde gelişimsel geriliklesonuçlanabilen merkezi sinir sistemi kalıcı hasarının önemli bir sebebidir. Bu çalışmadahastanemiz yenidoğan yoğun bakım servisinde 2007-2009 tarihleri arasında hipoksik iskemik ensefalopati tanısı ile yatırılarak tedavi görmüş olan 11 hasta incelendi.Gereç ve Yöntem: Olguların doğum öyküleri, annelerin gebeliklerinde gözlenen durumlar,apgar skorları, kan gazları, neonatal resüsitasyon ihtiyaçları, doğum ağırlıkları, cinsiyetleri,mekanik ventilasyon ihtiyaçları, klinik bulguları, Sarnat ve Sarnat evreleri, ek organ tutulumları, konvülziyon oranları, EEG ve görüntüleme sonuçları, hastanede yatış süreleri veçıkış durumları incelendi. Bulgular: Hipoksik iskemik ensefalopati sıklığı tüm doğumlarda %0,41, serviste yatanhastalarda %1,33 olarak belirlendi. Hastaların yedisinde %63,6 konvülziyon gözlendi.Sarnat ve Sarnat evrelemesine göre hastaların yedisi %63,6 Evre-II, üçü %27,2 Evre-Ive biri %9,1 Evre-III olarak değerlendirildi. Olguların biri %9,1 eksitus oldu. Sonuç: Daha önceki bilgilerden ve yayınlanmış literatürlerden farklı olarak hastanemizmortalite oranının oldukça düşük olduğu, bununda izlenen olgu sayısının azlığı ve de hastanenin hizmet verdiği özel popülasyondan kaynaklandığı düşünüldü. Bu çalışmanınülkemizde yayınlanan diğer çalışmalarla ve Sağlık Bakanlığına bağlı hastanelerin de katıldığı geniş bir araştırma ile tekrar değerlendirilmesi uygun olacaktır

A-Three-Year Short-Term Clinical Courses of Newborns with the Diagnosis of Hypoxic Ischemic Encephalopathy in a Tertiary Hospital

Introduction: Despite all improvements in neonatal intensive care practices, hypoxicischemic encephalopathy is an important cause of permanent injury of central nervoussystem leading to neonatal death, cerebral palsy and developmental retardation in thefuture. In this retrospective study, 11 patients who had been admitted to our hospital’sneonatal intensive care unit between years 2007 and 2009 with a diagnosis of hypoxicischemic encephalopathy were investigated. Materials and Method: Birth history, special occasions in pregnancies, apgar scores,blood gases, neonatal resuscitation needs, birth weights, gender, mechanical ventilationneeds, clinical states, Sarnat and Sarnat stages, additional organ involvements, convulsionrates, EEG and other imagining results, duration of hospital stays and hospital dischargestates of cases were investigated. Results: Incidence of hypoxic ischemic encephalopathy in all live births was 0.41% and1.33% in newborns admitted to hospital. Convulsions were seen in seven patients 63.6% . According to Sarnat and Sarnat Staging System; seven cases 63.6% were at Stage-II, three 27.2% at Stage-I andone 9.1% at Stage-III. One of the cases died 9.1% . Conclusions: In contradiction to previous informations and literature, we found that our hospital’s mortality rate was quite lowand this was tought to be due to minority of our cases and hospital’s special patient population. We think that it would be better to re-evaluate this study and other studies published in our country together within a broad investigatiton including
