Commentary on Prognostic Scoring Systems for Septic Patients in the Emergency Department

Objective: This letter aims to contribute to the original study by discussing the strengths and limitations of the MEDS score in septic patients. Materials and Methods: Findings from the article titled “Comparison of MEWS, qSOFA, and MEDS Scores in Predicting the Prognosis of Septic Patients in the Emergency Department” were reviewed in light of the literature. Results: The MEDS score shows high sensitivity but may be difficult to apply in emergency settings due to its complexity. Increasing the score for lower respiratory tract infection is a noteworthy suggestion. Statistical comparison using the DeLong test may strengthen conclusions. Conclusion: The MEDS score appears promising for early risk assessment in sepsis. However, its simplification and clinical validation through future studies are recommended.

Acil Serviste Sepsisli Hastaların Prognozuna Yönelik Skorlama Sistemleri Üzerine Yorumlar

Öz

Amaç: Bu yazı, sepsisli hastalarda kullanılan MEDS skorunun avantajlarını ve sınırlılıklarını tartışarak, mevcut makaleye katkı sağlamayı amaçlamaktadır. Gereç ve Yöntem: “Acil Servisteki Sepsisli Hastaların Prognozunda MEWS, qSOFA ve MEDS Skorlarının Karşılaştırılması” başlıklı çalışmanın verileri literatür eşliğinde değerlendirilmiştir. Bulgular: MEDS skoru yüksek duyarlılığı ile öne çıkmaktadır. Ancak karmaşık yapısı, acil serviste pratik kullanımı sınırlayabilir. Alt solunum yolu enfeksiyonuna verilen puanın artırılması önerisi dikkat çekicidir. Skorlar arası farkların DeLong testiyle analiz edilmesi metodolojik katkı sağlayabilir. Sonuç: Çalışma, MEDS skorunun sepsiste erken risk değerlendirmesinde faydalı olabileceğini göstermektedir. Ancak skorun sadeleştirilmesi ve klinik uygulamasının artırılması yönünde ileri çalışmalara ihtiyaç vardır.
