Comparison of the Effects of Low-Flow and High-Flow Sevoflurane Anesthesia on Hemogram Parameters in Patients Undergoing Septorhinoplasty

Öz

Objective: Surgical procedures and anesthesia can influence perioperative parameters by triggering a systemic inflammatory response. This condition may also alter perioperative hematological values in individuals. Inflammatory indices calculated from hemogram analysis, such as the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII), have increasingly been used to evaluate perioperative inflammation. However, the effects of different fresh gas flow strategies applied during anesthesia on these parameters remain unclear. Material and Methods: In this retrospective study, the data of 42 ASA I–II patients who underwent septorhinoplasty and received either low-flow (1–2 L/min) or high-flow (2–4 L/min) sevoflurane anesthesia during the operation were analyzed. Preoperative and postoperative 24-hour hemogram parameters and hemogram-derived inflammatory indices were compared between the groups. In addition, perioperative hemodynamic and respiratory parameters were also evaluated. Results: When the demographic characteristics of the patients were examined, no differences were observed between the groups. Hemogram parameters, including NLR, PLR, and SII, did not show significant differences between the low-flow and high-flow anesthesia groups in the preoperative and postoperative evaluations. Although postoperative increases in inflammatory indices tended to be higher in the high-flow group, these differences were not statistically significant. Hemodynamic and respiratory parameters throughout the procedure were also similar between the groups. The findings suggest that low-flow anesthesia does not increase perioperative inflammatory responses compared with high-flow anesthesia and provides comparable physiological stability. Conclusion: Low-flow sevoflurane anesthesia appears not to have adverse effects when hemogram and hemogram-derived inflammatory parameters are considered, and based on this, we believe that low-flow anesthesia can be regarded as a safe alternative to high-flow anesthesia in terms of inflammation.
