The effectiveness of interventions to improve immunosuppressive adherence of renal transplantation patients: Systematic review

Böbrek nakli yapılan hastaların immunsupresif ilaç uyumunu arttırmaya yönelik yapılan uygulamaların etkinliği: Sistematik derleme

Öz

Amaç: Böbrek nakli yapılan hastalarda ilaç uyumunu arttırmaya yönelik uygulamaların etkinliği hakkındaki bu sistematik literatür incelemesinin amacı, çalışmaların metodolojik kalitesini değerlendirerek ve uygulamaların içeriğini açıklayarak literatürün eleştirel bir değerlendirmesini sağlamaktır.

Gereç ve Yöntem: Konuyla ilgili olarak 2007-2019 tarihleri arasında yayınlanmış 5632 çalışma incelenmiştir.10 çalışma araştırmaya dahil edilme kriterlerine uygun olduğu için çalışma kapsamına alınmıştır. Tüm yayınlar çalışma yöntemleri, bulguları açısından sistematize edilerek derlenmiştir.

Bulgular: 10 çalışma incelenmek üzere araştırma kapsamına alınmıştır. Bu çalışmalardan 6’sı
Randomize Kontrollü Çalışma (RKÇ), 2 tanesi Retrospektif, 2’si ise Sistematik İnceleme kapsamındadır. Çalışmalar incelendiğinde deney gruplarına genel olarak bilişsel/eğitsel, danışmanlık/davranışsal, psikolojik/duygusal, sosyal ve finansal yaklaşımların kombinasyonları kullanılırken, kontrol grupları standart bakım almıştır. İlaç uyumunu arttırmak için kullanılan yöntemlerin uygulama yollarından, yüz yüze eğitim, telefonla görüşme, ev ziyareti, elektronik hatırlatma, akıllı ilaç şişeleri kullanılan yöntemlerden bazılarıdır.

Sonuç: Bu derlemenin bulguları, davranışsal risk faktörlerini hedef alan veya davranışsal, eğitsel ve duygusal uygulamaların kombinasyonunun ilaca uyumun arttırılmasında etkili olduğunu göstermektedir. Hastaların gelişim sürecine katılmaları teşvik edilirse, uygulamanın
etkinliği daha da artırılabilir.
