Enucleation Techniques In Prostate Surgery

Abstract

Benign
prostate hyperplasia (BPH) is a common  urologic disease  that lows the quality in men's life.
Especially after the 4th decade of life the prevelance of BPH is
increasing. If the medication is not enough 
in the treatment  than  suitable surgical procedures must be  recommended to the patients. In present there
is  different surgical techniques
performing. The European Assocaiation of Urology guideline  recommends 
Transurethal Resection of Prostate (TUR-P)  as a gold standart for men with prostate
sizes of 30-80 ml. When the prostate size is over than 80 ml.  open prostatectomy  or  endoscopic  enucleation of prostate  such as holmium laser or bipolar enucleation
are the first choices. But open prostatectomy has high perioperative and
postoperative morbidity.  
In
this review our goal is eveluating the endoscopic enucleation techniques of
prostate which begin to replace of open prostatectomy  in the treatment of BPH over 80 ml.

Prostat Cerrahisinde Enükleasyon Teknikleri

Abstract

Benign
prostat hiperplazisi (BPH) erkeklerin hayat kalitesini azaltan ve sık görülen
ürolojik bir hastalıktır. Özellikle hayatın 4. dekatından sonra görülme prevelansı
artmaktadır.  Tedavide ilaçlardan yeterli
yanıt alınamıyorsa mutlaka hastalara uygun cerrahi yöntemler önerilmelidir.
Günümüzde çeşitli cerrahi teknikler uygulanmaktadır. Avrupa Üroloji Derneğinin
kılavuzunda 30-80 ml arası prostat hacimlerinde altın standart cerrahi tedavi
olarak  Transüretral Prostat Rezeksiyonu
(TUR-P) önerilmektedir.  Prostat
hacimleri 80 ml. üzerinde olan hastalar için ilk seçenek açık prostatektomi, holmiyum
lazer  ve ya  bipolar enükleasyon gibi  endoskopik yöntemlerdir. Fakat açık
prostatektominin yüksek oranda peri ve postoperatif morbiditeleri
bulunmaktadır. 
Bu derlemenin amacı, BPH' nin
cerrahi tedavisinde 80 ml üzeri prostatlarda uygulanan açık prostatektominin
yerini almaya başlamış  enükleasyon tedavi şekillerini incelemektir.
