ERCP’de Koledokta Kız Veziküllerin Görülmesi İle Tanı Konulan Bir Kist Hidatik Olgusu

Öz

Kist hidatik insanlarda en sık echinococcus granulosus enfeksiyonu ile ortaya çıkan endemik bir hastalıktır. Çoğunlukla karaciğer (%60-
70) ve akciğeri (%20-30) tutmakla birlikte vücutta her organda görülebilir. Hepatik kist hidatiğin sık ve ciddi komplikasyonlarından birisi
de safra yollarına rüptürü olup %5-25 oranında görülür. Klinik olarak safra yollarına rüptür asemptomatik olabilirken, safra yollarına bası
veya safra kanalı tıkanıklığına sekonder kolesistit, kolanjit, karaciğer abseleri ve pankreatit gibi durumlarla da presente olabilir. Tanıda
USG en önemli tetkik olup safra yollarına rüptürü saptamada ise MRCP değerlidir. Safra yollarına rüptüre olan kist hidatik olgularında
preoperatifterapotik ERCP %80-100 oranında başarıyla uygulanarak kistobilier geçişler, hidatikmembranlar ve/veya bilier obstruksiyona
neden olan kız veziküller gösterilebilir. Bu yazımızda koledokolitiazis ön tanısıyla kliniğimize başvuran bir hastada yapılan ERCP de kız
veziküllerin görülmesiyle tanı konulan bir kist hidatik olgusunu sunmayı amaçladık.( Sakarya Tıp Dergisi 2016, 6(4):240-244 )

A Case of Cyst Hydatid which is Diagnosed By Appperarence of Girl Vesicles in Choledoc with ERCP

Hydatid cyst, an endemic disease in humans is most often caused by Echinococcus granulosus infection. In addition to being mostly
held in the liver (60-70%) and lungs (20-30%), it can occur in any organ of the body. One of the most common and severe complications
of hepatic hidatid cyst is also rupture into the biliary tract and it is seen in 5-25%. The rupture into the biliary tract can be clinically
asymptomatic; however, it can also occur with conditions such as bile duct pressure or cholecystitis secondary to bile duct obstruction,
cholangitis, liver abscesses and pancreatitis. While the most important examination for diagnosis is USG, MRCP is valuable for the detection
of rupture into the biliary tract. Biliary obstruciton caused by cysto-biliary transitions of Daughter vesicles and/or hydatid membranes
can be proven through implementation of preoperatively therapeutic ERCP success rate of 80-100% in patients with hydatid cyst
rupture into the biliary tract. Herein, we aimed to report a case of hydatid cyst diagnosed by the appearance of daughter vesicles during
ERCP performed on a patient admitting to our clinic with a presumptive diagnosis of choledocholithiasis.
