Çocukluk Çağında Yabancı Cisim Yutulması

Öz

Amaç: Çocukluk çağında sıklıkla karşılaşılan yabancı cisim yutmalarına yaklaşım standardize değildir. Kliniğimizde yabancı cisim yutması nedeni ile izlenen 1343 olgu sunularak deneyimlerimiz tartışılmıştır. Metod: Ocak 1999-Mayıs 2009 yılları arasında Dışkapı Çocuk Hastalıkları Eğitim ve Araştırma Hastanesi Çocuk Cerrahisi Kliniğine başvuran 1343 olgu hastane kayıtlarından geriye dönük olarak değerlendirilmiştir. Sonuçlar: Yabancı cisim yutan olguların 1264 (% 94.2)’sinde yabancı cismin gastroözofagial bileşkeyi geçtiği, 79 (%5.8) olguda ise özofagusta olduğu belirlendi. Olguların yaklaşık % 4.7’in de yutma anında retrosternal ağrı ve disfaji gibi bulgular mevcuttu. En sık yutulan yabancı cisim madeni para olarak belirlendi. Özofagusta bulunan yabancı cisimler rijid özofagoskop yardımı ile sorunsuz çıkarılırken, özofagogastrik bileşkeyi geçen yabancı cisimlerin %98’i kendiliğinden dışkı ile vücuttan atıldı. Onüç olgumuzda ise duodenum da takılmış olan açık çatal iğneler cerrahi veya endoskopik girişim ile çıkartıldı. Tartışma ve Sonuç: Özofagusta bulunan yabancı cisimler hemen çıkartılırken, gastroözofagial bileşkeyi geçen yabancı cisimlere yaklaşım yabancı cismin cinsine, büyüklüğüne ve yerleşim yerine göre değişmektedir. Özofagustan mideye ulaşan yabancı cisimlerin %98’i kendiliğinden dışkı ile vücuttan atılırken, %2’lik kısmına girişimde bulunmak gerekmiştir. Duodenumu geçmemiş olan yabancı cisimlerin dört gün süre ile lokalizasyonlarında değişiklik yok ise öncelikle endoskop yardımı ile noninvazif olarak çıkartılmaları planlanmalıdır.

FOREIGN BODY INGESTION IN CHILDHOOD

Öz

Objective: Although ingestion of foreign bodies are encountered frequently in childhood, the management is not standardized. We analyzed the management of 1343 cases of foreign body ingestion in children.Methods: The 1343 cases of ingestion foreign bodies admitted to our clinic in between 1999 to 2009 have been evaluated.Results: In 1264 patients (92.5%) the ingested foreign body had passed beyond the gastroesophageal junction. 4.7% of the patients had transient symptoms at the moment of ingestion, such as retrosternal pain and dysphagia. The most frequently ingested foreign bodies were metal coins. Foreign bodies in the esophagus were removed by rigid esophagoscopy in 79 patients. 98% of the foreign bodies were defecated after passing from esophagus to gastrointestinal tract without any incident. In 13 patients open safety pin could not pass the duodenal angulation and all of them were removed by endoscopy or surgery.Conclusion: The nature and localisation of the foreign bodies are important for the therapatic measures. Foreign bodies in the esophagus must be removed because of risk of obstruction and erosion of mucosa. If the foreign bodies do not leave the stomach in four days, it should be removed by endoscopy or surgery
