Clinical approach to the patient with epistaxis

Öz

Aim: Epistaxis refers to nasal bleeding of any cause. It is usually self-limited, but it may be severe and life threatening. Epistaxis is the most common cause of ear nose and throat (ENT) emergencies. It is not a disease but is an important symptom related to nose. An estimated 60% of the population will have a nosebleed in their lifetime, and 6% of these patients will require medical intervention. Only 10% of patients require hospitalization for recurrent epistaxis or accompanied vascular complications. In this study, we analyzed patients with epistaxis who required medical or surgical intervention.

Material and Methods: We evaluated the hospital records of 340 patients with epistaxis who admitted to our department during the last 2 years. All patients were initially examined by classical anterior rhinoscopy and endoscopic rhinoscopy. Patients, who didn`t respond to conservative therapy, underwent nasal packing, cauterization, and/or surgical management.

Results: One hundred thirty eight (40.5%) patients were female and 202 (59.5%) were male. 172 (50.5%) patients were given medical treatment, 116 (34.2%) patients were applied anterior nasal packing, 2 (0.5%) patients were applied posterior nasal packing, and 5 (1.5%) patients underwent surgery. 182 (53.6%) patients were between 2-20 years old, 104 (30.7%) patients were between 20-50 years old, and 54 (15.7%) patients were over 50 years of age.

Conclusion: Epistaxis is one of the most common otolaryngology emergencies. If the bleeding source is not identified by anterior rhinoscopy, a nasal endoscopy is mandatory. Identification and cauterization of the bleeding point under endoscopic magnification becomes an effective and less invasive procedure, avoiding unnecessary aggressive surgical interventions.
