Genişlemiş spektrumlu beta-laktamaz (GSBL) pozitif E.coli’ye bağlı olarak beyin apsesi gelişen olgu

Öz

Beyin apseleri nadir görülen infeksiyonları olmasına karşın, mortalite oranı yüksek infeksiyonlarıdır. Beyin apseleri sıklıkla sinüzit veya orta kulak infeksiyonları gibi yakın komşu dokulardaki infeksiyonların yayılımı sonucunda gelişir. Ayrıca, piyojenik etkenlerin hematojen yayılımı sonucu gelişen uzak organ infeksiyonları da beyin apsesine neden olabilir. Bu yazıda 69 yaşında, altta yatan hastalığı olmayan bir erkek hastada Genişlemiş spektrumlu beta-laktamaz üreten E.coli’ye bağlı olarak gelişen beyin apsesi ve bakteriyemi bildirildi. Hastada beyin apsesi materyalinde ve kan kültüründe E.coli üredi. İnfeksiyon odağına yönelik yapılan ekokardiyografi ve ultrasonografik incelemelerde ve diş muayenesinde infeksiyon odağı saptanamadı. Hastanın serebella bölgedeki apsesi cerrahi apse drenajı ve 6 hafta süreyle meropenem tedavisi sonrasında geriledi.

Brain abcess case due to extended spectrum beta-lactamase producing E.coli

Öz

Although brain abscesses are rare infections, they have high mortality rate. Brain abscesses often develop as a result of the spread of infections in distant tissues, such as sinusitis or middle ear infections. In addition, distant organ infections due to hematogenous spread of pyogenic agents may also cause brain abscess. In this article, we report brain abscess and bacteremia due to extended-spectrum beta-lactamase-producing E.coli in a 69-year-old male patient with no underlying disease. E.coli was grown in the brain abscess material and blood culture of the patient. No focus of infection was detected in the echocardiography and ultrasonographic examinations and dental examination. The patient's abscess in the cerebella region regressed after surgical abscess drainage and 6 weeks meropenem treatment.
