Oksijen Tedavisi

Öz

İki yüzyıldan daha fazla bir zaman önce keşfedilen oksijen, bilim teknoloji ve klinik tıp üzerinde geri dönülmez bir etki yaratmıştır. Oksijen inhalasyonu birçok hastalığın tedavisinde kullanılmaktadır. Son yetmiş yılda gelişmiş oksijen sistemleri, modern yoğun bakım üniteleri ve evde uzun süreli oksijen tedavisi ile oksijenin klinik tıpta kullanımı ve önemi artmıştır. Oksijen tedavisinin doğru yapılabilmesi için doku hipoksisine neden olan patofîzyolojik temeller iyi bilinmelidir. Oksijenin atmosferden doku mitokondirisine taşınması için pulmoner, kardiyovasküler ve hematolojik sistemler birbirlerini tamamlayıcı bir şekilde çalışmaktadır. Doku hipoksisi hızla düzeltilmezse hücre ölümü ve organ yetmezliği gelişebileceğinden hipoksik hasta iyi değerlendirilmeli ve doğru tedavi bir an önce uygulanmalıdır. Çoğu klinik durumda PaO2 ve SaO2 ölçümü, oksijen tedavisinin başlatılması, izlenmesi ve ayarlanması için başlıca göstergelerdir. Uzun süreli oksijen tedavisi (USOT) sadece hipoksemik kronik obstrüktif akciğer hastalarında değil, hipoksemiyle seyreden diğer kronik akciğer ve kalp hastalıklarında da tedavinin ayrılmaz bir parçasıdır. USOT’un kronik solunum yetmezliğinin gözlemlendiği olgularda yaşam süresini uzattığı ve yaşam kalitesini yükselttiği gösterilmiştir. Bu olumlu etkiler hipoksik hastalarda günlük oksijen tedavisinin yeterli süre kullanımı koşuluyla sağlanabilmektedir. Bugün geniş klinik deneyime ve tıp eğitiminde oksijen tedavisinin endikasyonlannın, uygulama prensiplerinin öğretilmesine rağmen oksijen tedavisinin uygun veya yeterli oranda yapılmadığı gözlenmektedir. Bu derlemede oksijen tedavisiyle ilgili temel oluşturmak için doku oksijenasyonunun fizyolojisi, hipoksemi, akut oksijen tedavisi, uzun süreli oksijen tedavisi ve oksijen sunum cihazları ile ilgili bilgi verilmesi amaçlanmıştır. Oxygen Therapy: Review

Öz

The discovery of oxygen over two hundred years ago has an irreversible impact on science, technology and clinical medicine. The inhalation of oxygen is being used as a treatment for many diseases. In the last seventy years, by the help of utilization of advanced oxygen systems, modern intensive care units and home based long-term oxygen therapy (LTOT), the clinical usage and importance of oxygen is significantly increased. For a decent oxygen therapy, it is necessary to understand the patho-physiological basis of tissue hypoxemia. Pulmonary, cardiovascular and hematological systems are in supplemental labour for transportation of oxygen from atmosphere to tissue mitochondria. Since tissue hypoxemia may quickly lead to cell deaths and organ failures, the conditions of hypoxic patient should be well evaluated and proper treatment must be applied promptly. In most of the clinical cases, the measurement of PaO2 and SaO2 are primary indications for initiating, monitoring and regulating the oxygen therapy. LTOT is an inseparable part of treatment for patients not only with chronic obstructive lung disease but also with other chronic lung and heart diseases leading with hypoxemia. Although indications and administration principles of oxygen therapy have been instructed in both broad clinical experience and medical practice, it’s observed that oxygen therapy has not been performed appropriately or sufficiently. In this review, it’s aimed to present information about the physiology of tissue oxygenation, hypoxemia, acute oxygen therapy, LTOT and oxygen delivery systems in order to constitute a basis for oxygen therapy.
