AYNI EKSTREMİTEDE HUMERUS SUPRAKONDİLER VE CİSİM KIRIĞIYLA BİRLİKTE DİSTAL RADİUS KIRIĞI: BİR OLGU SUNUMU

Öz

ÖZET Üst ekstremiteyi ilgilendiren kırıklar çocuklarda çok sık görülmektedir. Ancak aynı ekstremi- tede hem humerus suprakondiler ve cisim kırığı hem de distal radius kırığının olması oldukça nadirdir.Merdivenden düşme sonucu sol humerus cisim ve Gartland Tip 2 suprakondiler kırıkla beraber sol radius distal kırığı olan 13 yaşında bir çocuk hasta sunulmaktadır. Hastada damar-sinir hasarı yoktu. Humerus cisim kırığı için açık redüksiyon ve plakla tespit yapıldı. Suprakondiler humerus ve distal radius kırıkları için ise kapalı redüksiyon ve çapraz K-telleri ile tespit yapıldı. 3 haftalık uzun kol ateli uygulamasından sonra pasif hareket başlandı. Teller 5 hafta sonunda çıkarıldı. Son takipte iyileşme sorunsuzdu. Humerus kırığı tespit edilen vakalarda, aynı taraf önkol ve el bileği kırık ihtimali açısından mutlaka detaylı bir şekilde değerlendirilmelidir. Dr. Murat ZÜMRÜT Dr. Mehmet SAĞLAMOĞLU

IPSILATERAL DISTAL RADIUS FRACTURE WITH SHAFT AND SUPRACONDYLAR FRACTURE OF THE HUMERUS: A CASE REPORT

Öz

Fractures about the upper extremity are more common in children. However, the shaft and supracondylar fracture of humerus with a fracture of the distal radius in the same extremity is extremely rare. We here report the case of a 13year-old boy admitted after a fall from the stairs. Plain radiographs revealed the shaft and Gartland type 2 supracondylar fracture of left humerus and also distal fracture of the left radius. No evidence of neurovascular deficit was present. Open reduction with plate fixation was applied to the shaft fracture of humerus. Closed reduction with pin fixation was performed to fractures of supracondylar humerus and distal radius. A long-arm splint was applied for 3 weeks, then passive motion was begun. Pins were removed after 5 weeks from surgery. Recovery was uneventful at final follow-up. In the cases diagnosed as humerus fracture, ipsilateral forearm must be carefully evaluated in terms of the possibility of fracture.
