Yenidoğan ve Çocuklarda Candida auris: Artan Dirençli Bir Nozokomiyal Tehdit ve Güncel Tedavi Yaklaşımları

Öz

Son yıllarda çoklu ilaç direnci ve diğer candida türlerinden daha fazla çevresel dayanıklılığı nedeniyle Candida auris, çocuk ve yenidoğan yoğun bakım ünitelerinde gittikçe önem kazanan bir nozokomiyal patojen haline gelmiştir. Erken tanı güçlüğü, biyofilm oluşturma yeteneği ve ilaç dirençi, tedavi sürecini zorlaştırmaktadır. Prematürite, uzun süreli yoğun bakım yatışı, invaziv girişimler ve geniş spektrumlu antibiyotik kullanımı başlıca risk faktörleri arasında yer almaktadır. Güncel tedavi yaklaşımında ekinokandinler çocuklarda, amfoterisin B yenidoğanlarda ilk tercih olarak önerilmektedir. Yeni antifungal ajanlar ve moleküler düzeyde direnç mekanizmalarının anlaşılması, C. auris enfeksiyonlarının kontrolü için umut verici gelişmelerdir.

Candida auris in Neonates and Children: An Increasing Resistant Nosocomial Threat and Current Therapeutic Approaches

Öz

In recent years, due to its multidrug resistance and greater environmental resilience compared to other Candida species, Candida auris has become an increasingly important nosocomial pathogen in pediatric and neonatal intensive care units. Difficulty in early diagnosis, biofilm-forming ability, and drug resistance complicate the treatment process. Prematurity, prolonged intensive care stay, invasive procedures, and the use of broad-spectrum antibiotics are among the main risk factors. In the current treatment approach, echinocandins are recommended as the first choice for children, while amphotericin B is suggested as the first option for neonates. The development of new antifungal agents and the understanding of resistance mechanisms at the molecular level are promising advances for the control of C. auris infections.
