Use of electrolyte containing polyethylene glycol in childhood constipation Original Article

Abstract

Aim: To investigate the effectiveness and safety of polyethylene glycol nbsp; PEG 3350 as a therapy in children after the age of one year Material and Method: The study included 50 cases aged between 1 12 years All of the cases were prescribed PEG 3350 We aimed one or two painless soft bowel actions per day as the effectiveness of therapy We filled a form including every patient’s history and physical examination Complete blood counts with differential liver and renal function tests electrolytes of each patient were recorded All cases were evaluated every month while on PEG 3350 Wilcoxon test was used for comparing values before and after the therapy Results: The study included cases whose mean age was 4 6 years The mean duration of constipation was found 16 8 months Eight cases 16 had bloody stools seven cases nbsp; 14 had abdominal pain 35 cases 70 had pain with defecation 15 cases 30 had encopresis nbsp; besides the constipation complaint Thirty patients 60 had another pharmacologic treatment All children were prescribed PEG 3350 with electrolytes The effective dose which means one or two painless soft defecation was found at a mean dose of 0 75 gr kg day The time for resolution of constipation was 10 4 days averagely The average duration of theaphy was 5 5 months When we compared bowel movements per day before and after theraphy we found a significant differance Conclusions: Low dose PEG 3350 with electrolytes 0 65 0 75 gr kg day is safe and effective in childhood constipation Turk Arch Ped 2008; 43: 135 8 Key words: Child functional constipation polyethylene glycol with electrolytes

Öz

Amaç: Bir yaşından büyük çocuklarda tedavide elektrolit içeren polietilen glikol PEG 3350 kullanımının etkinliği ve güvenilirliğini araştırmak Gereç ve Yöntem: Çalışmaya yaşları 1 ile 12 yaş arasında olan 50 olgu alındı Olguların hepsine elektrolit içeren polietilen glikol tedavisi başlandı Tedavi etkinliği olarak ağrısız yumuşak dışkılama amaçlandı Her hasta için başvuru esnasında öykü ve fizik muayeneyi içeren bilgi formu dolduruldu Tam kan sayımı karaciğer ve böbrek işlev testleri ile elektrolit değerleri izlendi Olgular ilaç aldıkları sürede ayda bir klinik olarak değerlendirildi Tedavi öncesi ve sonrası ölçümsel verilerin karşılaştırılmasında Wilcoxon testi kullanıldı Bulgular: Olguların yaş ortalaması 4 6 yıl idi Kabızlık süre ortalamaları 16 8 ay bulundu Kabızlık yakınmalarına ilave olarak sekizinde 16 kanlı dışkılama yedisinde 14 karın ağrısı 35’inde 70 ağrılı dışkılama 15’inde 30 dışkı kaçırma vardı Elli olgunun 30’u 60 daha önce ilaç tedavisi almıştı Tüm olgulara elektrolit içeren PEG 3350 başlandı Tedavide günlük bir veya iki ağrısız yumuşak kıvamlı dışkılamanın sağlandığı etkin doz ortalama 0 75 gr kg gün olarak bulundu Kabızlığın düzelme süresi ortalama 10 4 gün idi Elektrolit içeren PEG 3350 kullanma süresi ortalama 5 5 ay idi Tedavi öncesi haftalık dışkı sayısı ve dışkı kıvamı ile tedavi sonrası haftalık dışkı sayısı ve kıvamı karşılaştırıldığında ileri düzeyde anlamlı fark vardı Olguların hepsinde tam klinik yanıt alındı nbsp; Çıkarımlar: Elektrolit içeren PEG 3350 düşük doz kullanılarak 0 65 0 75 gr kg gün güvenli ve etkin bulundu İlacın iki yaş ve altında da klinik ve laboratuvar izlemle kullanılabileceği izlenimi alındı Türk Ped Arş 2008; 43: 135 8 Anahtar kelimeler: Çocuk elektrolit içeren polietilen glikol işlevsel kabızlık
