Objective: To describe the epidemiological features of patients diagnosed with type 1 diabetes mellitus in the West Black Sea Region Material and Methods: Between 1998 and 2003 we analyzed the epidemiological features of 46 patients aged between 6 and 18 years 24 boys 22 girls who had type 1 diabetes mellitus The distributions of age gender season of birth and diagnosis and clinical signs and symptoms at the time of diagnosis were interpreted The patients were assigned to four groups according to their clinical and biochemical findings at the time of diagnosis The four groups were hyperglycaemia without ketones or acidosis ketotic patients without acidosis full diabetic ketoacidosis and finally diabetic coma The family histories of the patients were interpreted Results: Among 46 patients 26 patients were diagnosed primarily at the Department of Pediatrics Duzce Medical School and 20 patients were diagnosed by another research hospital and referred to our clinic during the follow up period The age at the time of diagnosis showed two peaks the first one between 6 8 years of age and the second between 10 12 years of age There was no significant difference between gender distributions The distribution of birth dates according to season sho wed that 27 58 patients were born in winter 9 20 patients in spring 6 14 patients in summer and 4 8 patients in autumn Consanguinity between parents was found in 12 26 patients Type 1 diabetes mellitus was diagnosed in the first degree relatives of 6 13 patients Ten 22 patients were diagnosed with hyperglycemia alone 17 37 patients with ketosis without acidosis 15 33 patients with ketoacidosis and 4 8 patients with ketoacidotic coma Only twentyfour 52 patients had health insurance Conclusions: In the West Black Sea Region in Turkey the age distribution at the time of diagnosis showed two peaks which one of them was between the ages of 6 and 8 years and the other between the ages of 10 12 years The early peak of presentations of diabetes 6 8 years may indicate that environmental factors such as viral infections and bottle feeding by cow’s milk in early life are causative factors in the development of type 1 diabetes mellitus It will require a large population study to clarify this point Key words: West Black Sea Region diabetes epidemiology
Amaç: Batı Karadeniz Bölgesinde tip I diyabetes mellitus tanısı ile izlenen çocukların epidemiyolojik özelliklerinin incelenmesi Gereç ve Yöntem: Bu çalışmada 1998 ve 2003 yılları arasında Düzce Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Bölümünde izlenen yaşları 6 ile 18 arasında değişen 46 24 erkek 22 kız hastanın epidemiyolojik özellikleri irdelendi Hastaların cinsiyete göre dağılımı doğum tarihlerinin ve tanı yaşlarının mevsimlere göre dağılımına bakıldı Hastalar tanı anındaki klinik ve biyokimyasal bulgularına göre dört gruba ayrıldı Bu dört grubu ketonemi veya asidoz olmadan hiperglisemi asidozis olmadan ketozis diyabetik ketoasidozis ve diyabet koması tablosunda tanı alanlar oluşturdu Hastaların aile öyküleri değerlendirildi
