Comment on: Pan-immune-inflammation value and platelet indices as mortality markers in intensive care patients with sepsis

Öz

Sepsis remains a leading cause of morbidity and mortality worldwide and represents a major diagnostic and prognostic challenge in the emergency department, where early risk stratification is essential for timely clinical decision-making. In this letter, we comment on the recently published study by Çetin et al., which evaluated the prognostic value of the Pan-Immune Inflammation Value (PIIV) and platelet indices, particularly mean platelet volume (MPV), in intensive care unit patients with sepsis. The authors demonstrated that both PIIV and MPV were independently associated with 30-day mortality, highlighting the potential clinical utility of composite inflammatory indices derived from routine complete blood count parameters. From an emergency medicine perspective, biomarkers that are rapidly available, low-cost, and easily calculated may contribute significantly to early assessment and disposition decisions, including intensive care unit admission. While the reported discriminative performance of PIIV and MPV was moderate, these findings provide a valuable foundation for further investigation. Prospective studies including patients at the time of emergency department presentation, as well as analyses of dynamic changes in these indices, may help clarify their role in early sepsis management. Overall, this study adds to the growing evidence supporting the use of composite inflammatory markers across the continuum of sepsis care.
