Deep neck space infections; our clinical experiences in a tertiary reference hospital

Öz

Objectives: Deep neck infections are clinical conditions that require early diagnosis and treatment due to their clinical manifestations, which can lead to serious morbidity and mortality. This study aims to investigate the de- mographic data and clinical outcomes of patients diagnosed with deep neck infections and treated at our clinic.
Methods: Data from 80 patients diagnosed with deep neck infections and treated at our clinic between 2015 and 2021 were retrospectively analyzed. Patient demographics, complaints, etiologies, blood tests, imaging methods, affected neck regions, culture results, treatment protocols, and hospital stay were recorded. The results were com- pared with the literature.
Results: % 36.25 of the patients were female and % 63.75 were male, and the median age was 41 years. The most common etiological cause was odontogenic infections. The median hospital stay was 6 days. The most common presenting complaints were neck swelling and pain. The submandibular area was the most affected area. Culture results revealed Staphylococcus epidermidis and Streptococcus anginosus as the most common pathogens. Sulbac- tam-ampicillin and metronidazole combinations were the most preferred antibiotics for treatment.
Conclusion: Deep neck infection is a treatable infection that can progress rapidly and lead to fatal complications. This study highlights the importance of early diagnosis and a multidisciplinary approach in the management of deep neck infection.

Derin boyun enfeksiyonları; üçüncü basamak referans hastanede klinik deneyimlerimiz

Öz

Amaç: Derin boyun enfeksiyonları, ciddi morbidite ve mortaliteye varabilen klinik manifestasyonu nedeniyle, erken tanı ve tedavi gerektiren klinik durumlardandır. Bu çalışmada, kliniğimizde derin boyun enfeksiyonu tanısı ile tedavi edilen hastaların, demografik verileri ve klinik sonuçlarının araştırılması amaçlanmaktadır.
Yöntem: Kliniğimizde, 2015-2021 yılları arasında derin boyun enfeksiyonu tanısı alarak tedavi edilen 80 hastanın verileri retrospektif olarak incelendi. Hastaların demografik bilgileri, şikayetleri, etiyolojileri, kan tahlilleri, görüntüleme yöntemleri, etkilenen boyun bölgeleri, kültür sonuçları, tedavi protokolleri ve hastanede yatış süreleri kayıt altına alındı. Sonuçlar, literatür ile karşılaştırılarak değerlendirildi.
Bulgular: Hastaların %36.25 'i kadın, %63.75 'i erkek olup, medyan yaş 41 olarak bulundu. En sık etiyolojik sebep odontojenik enfeksiyonlardı. Medyan hastanede yatış süresi 6 gün idi. En sık başvuru şikayeti boyunda şişlik ve ağrıydı. Submandibuler alan, en sık etkilenen bölgeydi. Kültür sonuçlarında en yaygın patojenler Staphy- lococcus epidermidis ve Streptococcus anginosus olarak saptandı. Tedavide sulbaktam-ampisilin ve metronidazol kombinasyonu en sık tercih edilen antibiyotiklerdi.
Sonuç: Derin boyun enfeksiyonu, hızlı ilerleyebilen ve ölümcül komplikasyonlara yol açabilen ancak tedavi edilebilir enfeksiyonlardandır. Bu çalışma, derin boyun enfeksiyonunun yönetiminde erken tanı ve multidisipliner yaklaşımın önemini vurgulamaktadır.
