Patient Safety in Terms of Infections Transmitted by Blood and Body Fluids

The hepatitis B, hepatitis C and human immunodeficiency viruses that spread through infected blood and body fluids result in infections that threaten the biosafety of health care workers and patients. These three viruses can spread through pathogens via direct contact with blood and body fluids with of sharp object wounds in health care settings. They are also at the forefront due to the severity of the infections they cause, their risks to public health, ethical and legal problems that they cause. The risks of this virus spreading from patients to health care workers or vice versa because of percutaneous wounds during medical interventions are affected by various factors including the type of the pathogen, the viral load of the source, the type of wound, and the volume of the blood. It is reported that 37% of hepatitis B virus infections, 39% of hepatitis C virus infections and 5% of human immunodeficiency virus infections in health care workers are caused by percutaneous wounds. Furthermore, the fact that these infections spread to patients through the invasive interventions conducted by health care workers is an under-examined topic worldwide. The United States, United Kingdom, and Australia are amongst the countries that have established guidelines to prevent the possible spread of diseases from health care workers to patients and to supervise this. These guidelines define the risky interventions concerning contamination, and the necessary criteria for the infected individual to carry out these interventions. Studies have revealed the insufficiencies of the occupational injury reporting process in our country. Additionally, the fact that all three viruses can be clinically silent and that there are no legal requirements for health personnel to have regular serologic screenings, shows that the risk of health care workers contaminating patients must be investigated in our country. In this study, we evaluated the international guidelines and the data in our country with regard to the transmission of hepatitis B, hepatitis C and human immunodeficiency virus from health care workers to patients. We aimed to draw attention to the need for national guidelines that protect the rights and privacy of health care workers by looking at the topic from medical, ethical, and legal perspectives. The preparation of guidelines that take into account the views of all related occupational organizations will both reduce the risk of health care workers contaminating patients during medical interventions and increase the health service quality in Turkey.

Kan ve Vücut Sıvıları ile Bulaşan Enfeksiyonlar Açısından Hasta Güvenliği

Öz

Enfekte kan ve vücut sıvıları ile bulaşan hastalık etkenlerinden hepatit B, hepatit C ve insan immun yetmezlik (HIV) virüsleri sağlık kurumlarında hem sağlık çalışanı hem de hasta biyogüvenliğini tehdit eden enfeksiyonlara neden olmaktadır. Sağlık merkezlerinde kesici delici alet yaralanmaları sonucunda kan ve vücut sıvılarına direkt temasla bulaşabilen patojenler arasında bu üç virüs, oluşturdukları enfeksiyonların ciddiyeti, halk sağlığını tehdit etmeleri ve bulaş gerçekleştiğinde ortaya çıkabilecek etik ve hukuki boyut nedeniyle de ön plana çıkmaktadır. Tıbbi girişimler sırasında perkutan yaralanmalar sonrasında gerek hastadan sağlık çalışanına gerekse çalışandan hastaya bu virüslerin bulaşma riskleri; patojenin tipi, kaynak vakanın viral yükü, yaralanmanın şekli ve bulaşan kan volümü gibi faktörlerden etkilenmektedir. Sağlık çalışanları arasındaki hepatit B virüs enfeksiyonlarının %37’ sinin, hepatit C virüs enfeksiyonlarının %39’unun, HIV enfeksiyonlarının ise yaklaşık %5’inin peruktan yaralanmalar sonucunda geliştiği bildirilmektedir. Öte yandan sağlık çalışanından uyguladığı invaziv girişimler sırasında bu enfeksiyonların hastalara da bulaşıyor olması tüm dünyada daha az irdelenmiş bir konudur. Amerika Birleşik Devletleri, Kanada, İngiltere ve Avustralya enfekte sağlık çalışanından hastaya olası bulaşmaların önlenmesine yönelik önlemleri ve denetimle ilgili adımları içeren rehberleri yazılı hale getiren ülkelerdendir. Bu rehberlerde bulaş açısından riskli invaziv girişimlerin tanımları yapılmış, enfekte çalışanın bu girişimleri yapabilmesi için sağlanması gereken kriterler belirlenmiştir. Yapılan çalışmalar ülkemizde mesleki yaralanma bildirimindeki yetersizlikleri ortaya koymaktadır. Bununla birlikte her üç etkenin klinik olarak sessiz seyredebiliyor olması ve sağlık personelinde düzenli serolojik taramalar için yasal bir zorunluluk olmayışı ülkemizde de sağlık çalışanından hastalara bulaşma ihtimalinin dikkate alınması gereken bir konu olduğunu düşündürmektedir. Biz bu derlemede sağlık çalışanından hastalara hepatit B, hepatit C ve HIV bulaşı ile ilgili uluslararası rehberleri ve ülkemiz verilerini inceleyerek, konuyu tıbbi, etik ve hukuki boyutuyla ele alırken çalışanın hak ve mahremiyetini de gözeten ulusal bir kılavuz oluşturulması gereksinimine dikkat çekmek istedik. İlgili meslek örgütlerimizin ortak görüşünü içeren bir rehberin hazırlanması gerek sağlık çalışanlarının gerekse hastaların riskini azaltarak Türkiye’de sağlık hizmetinin kalitesini de arttıracaktır.
