Auditory Brain Stem Responses Findings In Patients With Diabetes Mellitus And Atherosclerotic Hearth Disease

OBJECTIVE To determine whether the eighth cranial nerve and central nervous system damage occurs secondary to diabetes mellitus and atherosclerotic heart disease or not, in the subclinical period, using auditory brainstem responses (ABR). STUDY PLAN AND METHODS In this prospective study, study group consists of thirty-three patients with diabetes mellitus and thirty patients with atherosclerotic heart disease. Ten healthy volunteers comprised the control group. Only patients with type A tympanograms and with pure tone hearing thresholds of 0-15 dB between 500 and 8000 Hz were included in the study. Both ears were stimulated with 80 and 90 dB, ALT polarity clicks at a reputation rate of 10 clicks per second with a total of 2048 stimuli and ABR's were recorded. RESULTS I. and III. wave absolute latencies and I-III inter-peak latencies of patients either with diabetes mellitus or atherosclerotic heart disease were not statistically different from the control group. But, V. wave absolute latency and I-V and III-V inter-peak latencies were significantly prolonged in either patient group compared with the control group. CONCLUSIONS We conclude that ABR has a diagnostic value as a simple, low-cost and non-invasive method for determining the eighth cranial nerve and central nervous system damage in the subclinical period, occurring secondary to diabetes mellitus and atherosclerotic heart disease.

Diabetes Mellitus Ve Aterosklerotik Kalp Hastalığı Olan Hastalarda Uyarılmış İşitsel Beyin Sapı Yanıtları

Öz

AMAÇ Diabetes mellitus (DM) ve aterosklerotik kalp hastalığı (ASKH) seyrinde, 8. sinir ve santral sinir sisteminde hasar gelişip gelişmediğinin, subklinik evrede iken, uyarılmış işitsel beyin sapı cevapları (ABR) ile araştırılması. ÇALIŞMA PLANI VE YÖNTEMLER Bu prospektif çalışmanın gerecini 33 diabetli, 30 aterosklerotik kalp hastalığı olan hasta ve kontrol grubunu 10 gönüllü oluşturmuştur. Onyedisi erkek (%51), 16'sı kadın (%49) olan DM'lu, 24'ü erkek (%80), 6'sı kadın (%20) olan ASKH'lı ve 5'i erkek 5'i kadın, timpanogramları Tip-A ve odyogramda 500-8000 Hz frekans aralığında saf ses eşik ortalamaları 0-15 dB HL arasında olan hastalar çalışmaya alınmıştır. Her iki kulağa 80 ve 90 dB HL şiddette saniyede 10 tekrarlı ALT polaritede toplam 2048 klik sesli uyarı verilerek ABR ölçümleri yapılmıştır. BULGULAR Gerek DM'lu gerekse ASKH'lı olgularda I. ve III. dalga mutlak latansları ve I-III dalga pikleri arası latanslar açısından kontrol grubuna göre anlamlı fark bulunmaz iken, her iki hasta grubunda da santral ileti zamanını gösteren V. dalga mutlak latansı ve I-V ve III-V dalga pikleri arası latanslarda anlamlı derecede uzama saptanmıştır. SONUÇ DM ve ASKH seyrinde gelişen sekizinci sinir ve santral sinir sistemi hasarının subklinik evrede tespitinde basit, ucuz ve noninvaziv bir test olan ABR'nin tanısal değeri olduğu saptanmıştır.
