Physiology of Luteal Phase and its Modulation with Ovulation Induction Regimens

Luteal phase is the period between ovulation and menstruation and it is characterized by the formation of corpus luteum which produces progesterone. Progesterone secretion initiates the secretory transformation of endometrium and prepares it for implantation. Removal of the corpus luteum during early pregnancy results with abortion. Secretory transformation of endometrium is inadequate and shorter in duration in luteal phase defect which adversely affects the outcome of pregnancy. Ovarian stimulation protocols used mainly for in-vitro fertilization (IVF) lead to luteal phase problems and necessitate the progesterone support. In the current review, physiology of luteal phase in menstruel cycle, its modification by ovarian stimulation regimens, luteal phase defect and current approach for its support are summarized.
