Öz

Üretra, idrar ve semenin iletilmesinde görevli olup, bütünlüğü fonksiyonu için zorunludur. Travma, enfeksiyon vb. sebeplerle gelişen darlıklar işlev kaybına neden olur. Dilatasyon ve internal üretrotomi fazla invaziv olmasa da özellikle tekrarlayan olgularda başarısı düşüktür. Üretroplasti ise oldukça başarılıdır. Üretroplastide en sık kullanılan greft, avantajları sebebiyle oral mukozadır. Ancak, bazı durumlarda oral mukozanın kullanılamaması alternatif ihtiyacı doğurur. Bu noktada doku mühendisliği devreye girmektedir. Doku mühendisliği, iskele materyalleri, hücre kaynakları ve çeşitli faktörlerin kombinasyonu ile fonksiyonel doku üretimini hedefler. Sentetik ve doğal polimerler farklı biyomekanik özellikler sunarken, hücreselleştirilmiş greftler daha biyouyumludur. Yüksek maliyet, steril üretim ve zaman kısıtlaması gibi faktörler uygulamayı sınırlamaktadır. Sonuçta, klasik üretroplasti halen en çok tercih edilen yaklaşımdır. Ancak doku mühendisliğiyle üretilen greftler, karmaşık ve nüks eden olgularda, gelecekte cerrahi pratiğe katkı sağlayabilecek umut verici alternatifler sunmaktadır.
