Başlangıç Periodontal Tedaviye Ek Olarak Enjekte Edilebilir Trombosit Açısından Zengin Fibrinin Topikal Uygulamasının Klinik Etkinliğinin Araştırılması

Öz

Bu çalışmanın amacı, detertraj ve kök yüzey düzleştirmesine (SRP) ek olarak injekte edilebilir trombositten zengin fibrinin (i-PRF) periodontal klinik parametrelere kısa dönem etkisini araştırmaktır. Çalışmaya Evre II/III ve Derece B periodontitis ile teşhis edilen 23 hasta (32.17±8.14 yıl) dahil edilmiştir. Her hastanın bir kadranı kontrol grubuna (SRP), bir kadranı i-PRF grubuna (iPRF+SRP) rastgele atanmıştır. Klinik periodontal parametreler (Gingival indeks (GI), plak indeksi (PI), sondalanabilir cep derinliği (SCD), sondlamada kanama (SK), klinik ataçman kaybı (KAK), dişeti çekilme miktarı (DÇM)) ve dentin hassasiyeti başlangıçta (T0), 21. günde (T1) ve 3. ayda (T2) kaydedilmiştir. Elde edilen i-PRF, i-PRF grubundaki altı yüzeyinde de en derin ceplere sahip dişin cep tabanlarına enjekte edilmiştir. i-PRF uygulaması, T0 ve T1 zamanda olmak üzere toplam iki kere uygulanmıştır. Klinik parametreler açısından hem en derin cepte hem de aynı dişin altı yüzeylerine ait değerlendirmede her iki grupta da grup içinde zamanlar arasında anlamlı fark tespit edilirken (p<0.05), gruplar arası değerlendirmede anlamlı fark tespit edilmemiştir. Her iki grupta da PI, GI, SK, SCD, KAK zamanla azalırken, DÇM artmıştır. DH ise her iki grupta T1 zamanda artmış, T2 zamanda azalmıştır. Bu çalışmanın limitasyonları dahilinde başlangıç periodontal tedavinin her iki grupta iyileşme sağladığı ve i-PRF+SRP’nin, tek başına SRP’ye benzer şekilde klinik olarak olumlu bir etkiye sahip olduğu söylenebilir. Ancak klinik parametreler üzerinde ek bir üstünlük sağlamadığı gözlemlenmiştir.

Investigation of the Clinical Efficacy of Topical Application of Injectable Platelet-Rich Fibrin in Addition to Initial Periodontal Treatment

Öz

The aim of this study was to investigate the short-term effects of injectable platelet-rich fibrin (i-PRF) in addition to scaling and root surface planing (SRP) on periodontal clinical parameters. Twenty-three patients (32.17±8.14 years) diagnosed with Stage II/III and Grade B periodontitis were included in the study. One quadrant of each patient was randomly assigned to the control group (SRP), and one quadrant to the i-PRF group (i-PRF+SRP). Clinical periodontal parameters gingival index (GI), plaque index (PI), probing pocket depth (PD), bleeding on probing (BoP), clinical attachment loss (CAL), gingival recession (GR) and dentin hypersensitivity (DH) were recorded at baseline (T0), day 21 (T1), and month 3 (T2). The resulting i-PRF was injected into the pocket bases of the tooth with the deepest pockets on all six surfaces in the i-PRF group. i-PRF application was performed twice, at time points T0 and T1. In terms of clinical parameters, a significant difference was found within each group at different time points (p<0.05) for both the deepest pocket and the undersides of the same tooth, while no significant difference was found between the groups. In both groups, PI, GI, BoP, PD, and CAL decreased over time, while GR increased. DH increased at time point T1 and decreased at time point T2 in both groups. Within the limitations of this study, it can be said that initial periodontal treatment provided improvement in both groups and that i-PRF+SRP had a clinically positive effect similar to SRP alone. However, it was observed that it did not provide any additional superiority in terms of clinical parameters.
