Trust in the Health Care System, Health Services Utilization and Influencing Factors in Türkiye

Öz

Health systems rely on trust and accessibility. While the link between trust and health service utilization is recognized, evidence from Türkiye remains limited, making it challenging to address health system issues. This descriptive cross-sectional study includes 415 participants between August and September 2024. The mean healthcare utilization score is 7.45 ± 4.47, and the mean MTHCSS (Multidimensional Trust in Health Care Systems Scale) score is 3.31 ± 0.54. Pharmacies, hospital outpatient clinics, and family medicine services are the most used, while ambulances, hospitalizations, and private physician services are the least used. Continuous medication use significantly affects utilization, while age influences MTHCSS scores. No significant correlation is found between MTHCSS and utilization (r = 0.040, p = 0.416). Demographic factors play a role in both utilization and MTHCSS, highlighting their importance in evaluating health policies. Service preferences may help identify system burdens and guide resource allocation.

Öz

Sağlık sistemleri, güvene ve erişilebilirliğe dayanmaktadır. Güven ile sağlık hizmeti kullanımı arasındaki bağlantı vurgulanmış olsa da Türkiye'de bunun için yeterli kanıt yoktur. Bu kanıt eksikliği sağlık sistemi sorunlarını anlamayı ve iyileştirmeyi zorlaştırmaktadır. Araştırma, tanımlayıcı kesitsel tipte yürütülmüştür. Araştırma Ağustos-Eylül 2024 tarihleri arasında 415 katılımcı ile tamamlanmıştır. Ortalama sağlık hizmeti kullanım puanı 7,45 ± 4,47 ve ortalama MTHCSS puanı 3,31 ± 0,54'tür. Bireyler en çok eczaneleri, hastane polikliniklerini ve aile hekimliği hizmetlerini kullanırken; ambulansları, hastane yatışlarını ve özel hekim muayene hizmetlerini en az kullanmışlardır. Sürekli ilaç kullanımı sağlık hizmeti kullanımında etkili bir faktördür, yaş ise MTHCSS için etkili bir faktördür. Sağlık hizmeti kullanımı ile MTHCSS puanları arasında anlamlı bir korelasyon yoktur (r = 0,040, p = 0,416). Sonuçlar, farklı demografik faktörlerin sağlık hizmeti kullanımı ve MTHCSS üzerinde etkisi olduğunu göstermiştir. Bu, sağlık politikaları ve sağlık sistemleri analiz edilirken demografik faktörlerin dikkate alınması gerektiğini göstermektedir. En çok veya en az tercih edilen hizmetler, sağlık sistemlerinin yük altında olduğu alanların analizine katkıda bulunabilir. Bu, sağlık hizmetlerinin hangi alanlarına daha fazla kaynak ayrılması gerektiği konusunda rehberlik sağlayabilir.
