OVERVIEW OF CIRCUMCISION COMPLICATIONS WITH EPIDERMAL INCLUSION CYST CASES
Abstract
Background/ Aim: After the trauma or surgical procedures epidermal cysts, which occurred implantation of the superficial epidermal tissue to the dermis or subcutaneous tissue, can be seen in any part of the human body and they are real cystic formation., Perimeter of cysts are surrounded by keratinized squamous epithelium, and inside of it is filled with keratin debris. In this study, two patients with penile epidermal cysts growing after the circumcision surgery were presented.
Case 1: A 4 years old boy who had circumcised by a pediatric surgeon in another center 2 years ago, was consulted with a growing mass in penis for one year. There was a 1 cm diameter mass under the circumcision incision on the physical examination, it was mobil, and round and smooth contoured. Superficial sonography of the penil tissue showed a 10.7X10.4mm solid mass of penile tissue. Doppler US reported blood flow around the mass, but showed that there was no blood flow inside of the mass. Laboratory tests were normal, and the mass was excised without any rupture or complications. Histological examination was reported as inflamed epidermal cyst.
Case 2: A 2
years old boy who had circumcised at our department one year ago
admitted us with a penil mass. There was a 0.6 mm diameter noduler
polypoid lession at the level of circumcision insicion in frenular
area. It was mobil. Laboratory tests were normal, and lession was
excised totally without any complications. Histological examination
of the mass was reported as epidermal cyst.
Conclusion:
Circumcision should not be seen as
an ordinary process and it also must be done carefully by specialist
to prevent of children from repeated psychological and surgical
trauma. Circumcision is a real surgery performed on a healthy body
part. As all surgical operations, this surgical procedure has to be
performed with special attention for prevention of these and other
circumcision complications. To prevent of parents frustrations which
expressed with “But, a pediatric surgeon performed the
circumcision!" should be one of our main objectives.
