ERKEK REPRODÜKTİF FONKSİYONLARDA D VİTAMİNİNİN ROLÜ

Öz
Kalsiyum-fosfor homeostazının ve kemik sağlığının temel düzenleyicilerinden olan D vitamininin üreme organlarının da içinde bulunduğu bir takım klasik olmayan fonksiyonları tanımlanmıştır. D vitamininin erkek üreme fonksiyonlarında rol aldığına dair çok sayıda çalışma mevcuttur. Bu makalede D vitaminin erkek üreme fonksiyonları ile ilişkisini inceleyen çalışmalar derleme kapsamına alınmıştır. D vitamini karaciğer ve böbrekte metabolize olabildiği gibi erkek üreme organlarında da metabolize olmaktadır. Bu nedenle D vitamini reseptörü (VDR) ve 24-hidroksilaz enzimi semen kalitesi için pozitif markerlardır. Çalışmaların büyük bir çoğunluğu D vitamininin gonadal fonksiyonları modüle ettiğini göstermektedir. Bu etkinin direkt ya da indirekt olarak gen ekspressyonuyla ilişkili olabileceği düşünülmektedir. Normal serum D vitamini seviyelerinin sağlanması sperm fonksiyonlarının ve cinsiyet hormonlarının optimal düzeyde çalışmaları açısından önemlidir. Sperm fonksiyonlarıyla D vitamini ilişkisini değerlendiren çalışmalarda oligospermisi (spermatozoa sayısında azalma) ya da asthenozoospermisi (sperm motilitesinde azalma) olan, serum D vitamini yetersiz erkeklerde D vitamini suplemantasyonu önerilebileceği belirtilmektedir. Ancak suplemantasyon yapılırken D vitamininin yüksek dozlarda alınmasının yaratabileceği toksik etkilerin göz ardı edilmemesi gerekmektedir. D vitamini suplemantasyonunun erkek reprodüktif fonksiyonları üzerine etkilerinin daha iyi anlaşılabilmesi için daha büyük randomize kontrollü prospektif çalışmalara ihtiyaç duyulmaktadır.
