Fibromiyalji Hastalarında Tele Sağlık Müdahalesinin Semptom Yönetimi Üzerine Etkisi: Sistematik Derleme

Abstract

Amaç: Fibromiyalji; kronik ve yaygın kas ağrıları ile karakterize, uyku sorunları, yorgunluk, bilişsel problemler, sabah tutukluğu ve sindirim sorunları gibi birçok semptoma neden olan patogenezi tam olarak bilinmeyen kronik bir romatizmal hastalıktır. Tele sağlık uygulamalarının fibromiyalji gibi kronik hastalıklara sahip bireyler için özellikle semptom yönetimi sürecinde aktif şekilde kullanılmaya başlandığı görülmektedir. Bu sistematik derlemenin amacı, tele-sağlık müdahalesinin fibromiyalji hastalarında semptom yönetimine etkisini değerlendirmektir.
Yöntem: Çalışma için Cochrane Library, ProQuest Central, PubMed, Science Direct ve Google Akademik veri tabanları veri tabanları belirlenen anahtar kelimeler kullanılarak tarandı. Taramalar sonunda; İngilizce ve Türkçe dilinde, 2013-2023 yılları arasında yayınlanmış, tam metnine ulaşılan, dahil edilme kriterlerine uyan iki randomize kontrollü çalışma araştırma kapsamına alındı. Çalışmalardan birinde telefon desteği ile rutin bakım uygulamasının; diğerinde telefon danışmanlığı ile açıklayıcı video uygulamasının karşılaştırıldığı görüldü.
Bulgular: Araştırmaya dahil edilen bu iki çalışma içinde telefon müdahalesinin fibromiyalji semptomlarından ağrı, uyku, yorgunluk, depresyon ve duygu-durum düzeyi üzerinde etkileri incelenmiştir. Çalışmalar sonucunda tele sağlık müdahalesinin iki çalışma içinde fibromiyalji semptomlarından ağrı, uyku kalitesi ve depresyon düzeyi üzerinde etkili olduğu belirlenmiştir.
Sonuç: Fibromiyalji patogenezinin tam olarak anlaşılamadığı ve semptom kontrolünün zayıf olduğu göz önünde bulundurularak; semptom kontrolünün son derece önemli olduğu bu hastalıkta tele sağlık yöntemlerinden faydalanarak bakımın kalitesini arttırmak önem arz etmektedir. Bu kapsamda fibromiyalji hastaları ile daha geniş örneklem dahilinde, randomize kontrollü çalışmaların yapılması önerilmektedir.

Effect of Telehealth Intervention on Symptom Management in Fibromyalgia Patients: A Systematic Review

Abstract

Aim: Fibromyalgia is a chronic rheumatic disease of unknown pathogenesis characterized by chronic and widespread muscle pain, causing many symptoms such as sleep problems, fatigue, cognitive problems, morning stiffness and digestive problems. Telehealth applications are actively used especially in the symptom management process for individuals with chronic diseases such as fibromyalgia. The aim of this systematic review was to evaluate the effect of a telehealth intervention on symptom management in fibromyalgia patients.
Methods: The Cochrane Library, ProQuest Central, PubMed, Science Direct and Google Scholar databases were searched using the identified keywords. At the end of the screening, two randomized controlled trials published in English and Turkish between 2013 and 2023, whose full texts were accessed and which met the inclusion criteria were included in the study. One study compared telephone support with routine care and the other compared telephone counseling with explanatory video.
Results: In these two studies, the effects of telephone intervention on fibromyalgia symptoms such as pain, sleep, fatigue, depression and mood levels were examined. As a result of the studies, it was determined that telehealth intervention was effective on pain, sleep quality and depression levels among fibromyalgia symptoms in two studies.
Conclusion: In conclusion, considering that the pathogenesis of fibromyalgia is not fully understood and symptom control is poor; it is important to improve the quality of care by utilizing telehealth methods in this disease where symptom control is extremely important. Therefore, it is recommended to conduct randomized controlled trials with a larger sample of fibromyalgia patients.
