Ameliyat Öncesi Cilt Hazırlığı ve Antibiyotik Kullanımının Ameliyat Sonrası Yara Yeri Enfeksiyon Oluşumuna Etkisi

Abstract

Bu araştırmada ameliyat öncesi antiseptik solüsyonunun kullanımı, tüy temizleme şeklinin nerede, ne zaman yapıldığını ve profilaktik antibiyotik seçimini belirlemek amacıyla tanımlayıcı olarak yapıldı. Araştırma Ankara Üniversitesi Tıp Fakültesi Kalp Merkezinin ameliyathanesinden ve kalp damar cerrahisi servislerinden belirli zaman diliminde veriler toplandı. Hastaların bilgilendirilmesi ve ameliyat sonrası 3. Gün değerlendirilmesi kalp damar cerrahisi servisinde yapıldı. Hastaların tüy temizliği kontrolü ve ameliyat esnasında uygulanan yapışkan örtü, antibiyotik kullanımı ameliyathanede değerlendirildi. Evreni belli gruplarda örneklem seçimine göre örneklemi 60 kişi bulundu. Veriler Gönüllü Onam Formu, Tanıtıcı Bilgi Formu, Ameliyathanede Bakılacak Değerler Formu ve Ameliyat Yeri Enfeksiyon Değerlendirme Formu ile veriler toplandı. Elde edilen veriler; sayı yüzde, ortalama ile gösterildi ve ki kare testi ve t testi ile istatistiksel anlamlılıklarına bakıldı. Hastaların ameliyat öncesi cilt hazırlığında povidon iyodin kullanılması ve bazı hastalarda drape kullanımının devam etmesi CAE riskini istatiksel olarak etkilemediği tespit edildi (p>0.05). Hastaların kan glikozu regülasyonun yapılıyor olması nedeniyle CAE riski istatiksel olarak artmadı (p>0.05). Hastaların normotermik olması, sıvı volüm değerlerinin ve yaşam bulgularının normal sınırlarda olması, oksijenlenmede problemi olmayan hastaların olması yara yerinde CAE riskini azaltmış olabilir; ancak hastalarda albümin düzeyinin farklı olması CAE’nın varlığını istatistiksel olarak etkilemiştir (p<0.05). Daha büyük örneklemde CAE’nı etkileyecek tüm parametrelerin tekrar araştırılması önerildi.

The Effect Of Pre-Surgery Skın Preparatıon And Antıbıotıc Use On Post-Surgery Wound Infectıon

Abstract

In this study, the use of antiseptic solution before surgery was defined to determine where and when feather cleaning was provided and the choice of prophylactic antibiotics. Research data were collected from the operating room and cardiac surgery services of Ankara University Faculty of Medicine Heart Center over a certain period of time. The patients were informed and evaluated on the 3rd postoperative day in the cardiovascular surgery service. The patients' feather health check, adhesive dressing applied during surgery, and antibiotic use were evaluated in the operating room. According to the elections in the groups in the universe, the elections were 60 people. Data were collected with the Voluntary Consent Form, Introductory Information Form, Values to be Measured in the Operating Room Form and Surgery Site Infection Evaluation Form. The data obtained; Significance was examined using number, percentage, mean, and chi-square test and t-test. It was determined that the risk of SSI was not statistically affected by the use of povidone-iodine and the use of some flexible dressings in the preoperative skin conditions of the patients (p>0.05). The risk of SSI did not increase statistically due to the patients' blood glucose regulation (p>0.05). The risk of SSI at the wound site may be reduced if the patients are normothermic, their fluid volume values and survival rates are within normal limits, and there are no problems with oxygenation; However, the different albums were affected by the structures of SSI (p<0.05).
