ARAÇ İÇİ TRAFİK KAZASI KAYNAKLI ÖLÜM OLGULARINDA TRAVMATİK BULGULAR İLE ARAÇ İÇİ LOKALİZASYON VE KAZA TİPİ ARASINDAKİ İLİŞKİNİN DEĞERLENDİRİLMESİ; RETROSPEKTİF OTOPSİ ÇALIŞMASI

Öz

Amaç: Araç içi konumun önemli olduğu adli olguların çözümüne ve trafik kazalarının neden olduğu kayıplara yönelik önlemlere katkı sağlayacağı düşüncesinden hareketle bu çalışma planlanmıştır. Yöntemler: Adli Tıp Kurumu Başkanlığı Morg İhtisas Dairesi’ne Eylül 2007 - Nisan 2008 döneminde otopsi yapılmak üzere gönderilen; trafik kazasına bağlı 444 ölüm olgusu içinden araç dışı trafik kazası, motosiklet kazası, yurt dışından gelen ve bilgilerine ulaşılamayan trafik kazası olguları ayrılmış, olayla ilgili gerekli bilgilere ulaşılabilen toplam 150 araç içi trafik kazası sonucu ölüm olgusu retrospektif olarak değerlendirilmiştir. Bulgular: Olguların %83’ü n:125 erkek, %17’si n:25 kadındır. Trafik kazaları her iki cinsiyette de genç erişkin popülasyonunda önemli bir ölüm nedeni olarak saptanmıştır. Olguların %60,7’sinin sürücü, trafik kazalarına karışan araçların %86’sının otomobil olduğu, ölümlerin %63’ünün önden çarpışmalar sonucu ve en sık %44 oranında otoyolda meydana geldiği saptanmıştır. Kanında alkol saptanan 19 sürücünün 16’sında 0,5 promil olan yasal sınırın üzerinde etil alkol değerleri belirlenmiştir. Her iki cinsiyet için de kafa travması ve torakoabdominal yaralanmalar ölümde en sık etkili faktörler olarak belirlenmiştir. Sonuç: Kesin olarak kişilerin araç içi pozisyonunu söyleyebilmek için olay yeri incelemesine otopsiyi yapacak adli tıp uzmanının da katılarak cesetlerin olay yerindeki pozisyonlarını görmesi Ceza Muhakemeleri Kanunu CMK ’nun da verdiği yetkiyle olayın tanıklarıyla gerekli gördüğü noktalar üzerinden görüşmesi ve teknik bilirkişilerle görüş alışverişinde bulunmasının gerekli olduğunu düşünmekteyiz.

EVALUATION OF RELATIONSHIP BETWEEN ACCIDENT TYPES, LOCALIZATION OF OCCUPANTS IN THE VEHICLE AND INJURY PATTERN IN FATALITIES RESULTING FROM ROAD TRAFFIC ACCIDENT: RETROSPECTIVE AUTOPSY STUDY

Öz

Objective: In order to contribute to the efforts targeted to decrease the casualties of road traffic accidents, we planned this retrospective study and by this way intended to provide the wound patterns of these fatalities. Methods: There were total 444 fatalities resulting from road traffic accidents among the autopsy population of Council of Forensic Medicine, Mortuary Division between September 2007 and April 2008. The accidents involving the pedestrian and the cases having little or no information about the incident were excluded and remaining 150 fatalities were retrospectively evaluated. Results: Of the cases, 83% n:125 were male and 17% n:25 female and female/ male ratio was 1/5. Road traffic accidents were detected to be an important cause of death in young adults in both genders. Most cases were detected to be in the 21-30 age groups in both genders. Victims were the drivers in 60.7% of the cases and automobile was the most frequently engaged vehicle in fatal accidents. 63% of the accidents were frontal collisions and the most common site of the incident was motorway. Blood analysis for alcohol yielded the presence of alcohol in 20.9% n:19 of the driver n:91 and in 16 driver 17.6% the blood alcohol level exceeded the official level permitted 50mg/dl . The 21-30 age group which is the most crowded group was also the leading age group for alcohol presence. Head trauma and thoraco-abdominal injuries were detected to be most important injuries in both genders in these fatalities. Conclusion: In order to accurately define the sites of passengers in the car will definitely require the examination of the death scene and gathering of all the information from witnesses and technical staff by forensic doctor.
