Öz

Bir evli çiftin aynı anda ya da çok az ara ile ölümüne nispeten az rastlanmakta olup, karşılaşıldığında ilk akla gelen doğal olmayan ölüm sebepleridir. Ancak bazı durumlarda adli tahkikat, otopsi sonuçları, olay yeri detayları, kişilerin özgeçmişleri gibi faktörler göz önüne alınarak bazı çift ölümlerinin doğal orijinli olduğu düşünülmüştür ve bu vakalar kendilerine medikolegal terminolojik bir yer edinmiştir: “Philemon ve Baucis Sendromu”. Çalışmamızda evli olup, bir ildeki evlerinde ölü bulunan 2 olgu sunulmuştur. Her iki olgunun 80 yaşın üstünde olduğu, önceden bilinen kardiyak ve vasküler sorunları olduğu dikkat çekmektedir. Yapılan olay yeri incelemesi ve adli tahkikatta evde zehirlenmeye sebebiyet verecek herhangi bir gaz kaçağı olmadığı, ortam ısısında bir anormallik saptanmadığı görülmüş, partnerlerden herhangi birisinin bir diğerine bakım veren olmadığı, komşularının ve kiracılarının genellikle günlük ihtiyaçlarını karşılayarak yardımcı oldukları öğrenilmiştir. Altta yatan sağlık sorunlarının predispozan etkisiyle beraber diğer eşin ölümüne şahit olmanın akut psikolojik stres kaynağı olduğu ve ani kardiyak ölüm etkeni olabileceği de bilinmektedir. Çift ölümlerine her dönem rastlansa da doğal nedenli çift ölümleri literatürde kendisine son yıllarda yer bulmaya ve kaydedilmeye başlamıştır. Burada karşımıza çıkan en büyük zorluk durumun değerlendirilmesi ve nedensellik bağlarının kurulmasıdır. Bu çalışmada vaka sayılarının azlığı sebebiyle literatürde hali hazırda var olan “birlikte doğal ölüm” vaka sayılarına katkı sağlamak ve benzer vakalar eşliğinde tartışmak hedeflenmektedir.

Simultaneous Natural Death of an Elderly Couple- Philemon and Baucis Syndrome: A Case Report

Öz

The simultaneous or nearly simultaneous death of a married couple is relatively rare and when encountered, the primary considerations are usually unnatural causes of death. However, factors like forensic investigations, autopsy results, crime scene data and the individuals’ backgrounds have led to the consideration that certain couple deaths may be of natural origin. These cases have established a place in medicolegal terminology: “Philemon and Baucis Syndrome.” In our study, we present two cases of a married couple found deceased in their home in city. Notably, both cases involve individuals over the age of 80 with known cardiac and vascular issues. It is recognized that underlying health problems, coupled with the acute psychological stress of witnessing the death of a spouse, may act as a precipitating factor and could contribute to sudden cardiac death. While couple deaths have been observed throughout various periods, natural causes of couple deaths have only recently begun to be recognized and documented in the literature. The primary challenge encountered here, due to the low number of cases, is the assessment of the situation and establishing causal relationships. This study aims to contribute to the existing literature on “simultaneous natural deaths” by providing additional case numbers and discussing similar cases.
