SANTRAL DEV HÜCRELİ GRANÜLOMLARDA GEÇMİŞTEN GÜNÜMÜZE TEDAVİ YÖNTEMLERİ

Öz

Santral dev hücreli granülomlar (SDHG), çene kemiklerinde oldukça nadir görülen lezyonlardır. Geçmişten günümüze pek çok farklı isimlendirmeleri vardır ve etyolojileri hala net olarak bilinmemektedir. Radyolojik ve klinik özellikle açısından ameloblastoma, miksoma, cherubizm, anevrizmal kemik kisti vs. lezyonlarla ayırıcı tanısının yapılması gerekir. Ancak tanı koyulduktan sonra dahi uygun tedavi yönteminin seçilmesi her zaman kolay olmayabilir. Literatürde en çok bilinen ve uygulanan tedavi yöntemi küretaj olsa da agresif tipte seyreden lezyonlarda bu tedavi şekli nüks ve çene kemiklerinde ileri düzey defektlerle sonuçlanabilir. Bu sebepten dolayı intralezyonal steroid enjeksiyonu, denasumab, tirozin-kinaz inhibitörü, kalsitonin uygulamaları gibi tedavi seçenekleri geliştirilmiştir. Ancak optimal tedavi algoritması henüz tanımlanmamıştır. Bu derlemenin amacı; mevcut tedavi seçeneklerinin uygulama şekilleri, dozları, güncel literatürdeki vaka raporları ve çalışma sonuçları hakkında bilgi vermektir.

THE CURRENT TREATMENT METHODS IN CENTRAL GIANT CELL GRANULOMAS

Öz

Central giant cell granulomas (SDHG) are extremely rare lesions in the jaw bones. It has many different identifications from past to present, and their etiology is still not clearly known. In terms of radiological and clinical features, differential diagnosis should be made with lesions such as ameloblastoma, myxoma, cherubism, aneurysmal bone cyst etc. However, even after the diagnosis is made, it is not always easy to choose the appropriate treatment method. Although curettage is the most known and applied treatment method in the literature, this type of treatment may result in recurrence and advanced defects in the jaw bones in aggressive lesions. For this reason, different treatment options such as intralesional steroid injection, denasumab, tyrosine-kinase inhibitor, calcitonin applications have been developed. However, the optimal treatment algorithm has not yet been defined. The aim of this review was to investigate information about the application methods, doses of treatment methods and presented case reports and studies in the current literature.
