Öz

The clinical diagnosis of psychosis in Lacanian theory captures what is beyond the notions of a psychotic break and evaluates the phenomenon based on the relational dynamics between the subject and the Other. The main operation of psychosis that determines the clinical structure is the foreclosure of the Name-of-the-Father (the primordial signifier). The foreclosure of the signifier prevents the subject from entering the Symbolic register, which is considered as the basis of language distortions in psychosis. The subject does not have a means to connect the remaining registers: the Real and the Imaginary. In the absence of a signifier, the subject may adopt compensatory mechanism by finding imaginary identifications. As an attempt to find meaning, the subject can rely on the imaginary identifications before the triggering of delusions and hallucinations. In this article, firstly the Lacanian diagnostic concepts of psychosis were outlined, then the concept of untriggered psychosis was discussed in reference to a clinical case with imaginary identifications and language distortions and no report of hallucinations or delusions, and the treatment process was examined in light of Lacanian approach.

Lacanyen Tanıda Psikoz: Tetiklenmemiş Psikozun Klinik bir Vakada Tartışılması

Öz

Lacanyen teoride klinik bir tanı olan psikoz, psikotik kırılma kavramının ötesini içermekte ve bu klinik durumu kişinin Başka ile ilişkileri kapsamında değerlendirmektedir. Psikozun temel mekanizması olan Babanın Adı’nın (baş gösteren) hesaptan düşmesi psikotik klinik yapıyı belirlemektedir. Baş gösterenin hesaptan düşmesi kişinin simgesel düzene geçmesini engellemekte ve bu durum psikozda dil bozukluklarının meydana gelmesinin temel nedeni olarak nitelendirilmektedir. Kişi simgesel düzenin yokluğunda geriye kalan imgesel ve gerçek düzenlerini birleştirecek bir araca sahip değildir. Baş gösterenin yokluğunda kişi, imgesel özdeşleşmeler bularak bir nevi telafi etme mekanizmasına başvurmuş olur. Sanrıların ve hezeyanların tetiklenmesinden önce kişi, telafi etme mekanizmasını ancak bu bahsi geçen imgesel özdeşleşimlerle anlam bulabilme çabası içerisinde belirleyebilir. Bu makalede, öncelikle psikozun Lacanyen teorideki başlıca ögeleri sıralanmış, sonrasında tetiklenmemiş psikoz kavramı, imgesel özdeşleşmeleri ve dil bozuklukları mevcut olan, ancak sanrı ve hezeyan bulgusu saptanmamış bir vaka örneği çerçevesinde tartışılmış ve psikozdaki psikoterapi sürecinin nevrozdaki psikoterapiden farklılaşan yanları da ele alınarak psikoz Lacanyen yaklaşımın ışığında incelenmiştir.
