Metaphor Analysis for Holistic Nursing Care: Constructing a Puzzle

Background: In order for nurses to analyze what holistic nursing care is, which includes many concepts, it is necessary to make sense of the holistic care process, its components and effects. In this context, it is extremely important to determine how nurses perceive holistic care. Objectives: Research; the aim of this study was to reveal the meanings attributed by nurses to holistic nursing care through metaphors. Methods: Research; using a phenomenological (phenomenological) design, one of the qualitative research designs, was carried out with 227 nurses who agreed to participate in the research in a training and research hospital between March-April 2021. The data were obtained with a 10-question data collection form created by the researchers, 9 questions were asked in order to determine the demographic characteristics of the nurses and in order to determine the metaphors related to the concept of holistic care; ‟holistic nursing care ………. like this; because …………” sentence is required to be completed. The analysis of the data was evaluated with a five-stage content analysis. Results: Of the 227 nurses who agreed to participate in the study, it was seen that 209 nurses who filled out the form completely and had no inconsistency between the metaphor and the explanation produced 106 metaphors. These metaphors they created were examined in 9 categories: "reassuring, inclusive, helping, need/necessity, compulsory duty, professionalism, moral responsibility, unimportant/worthless, guiding/reflective". Nurses produced 89% positive metaphors. The metaphors most often produced by nurses are; mother (n = 22, 20.8%), puzzle (n = 15, 14.2%), rainbow (n = 6, 5.7%), sun (n = 5, 4.7%), family (n = 5, 4.7%). Conclucion: It has been determined that nurses have predominantly positive metaphors about the perception of holistic care. For each category of metaphors, strategy planning should be done in order to support and develop positive perceptions and to change negative perceptions.

Bütüncül Hemşirelik Bakımı İçin Metafor Analizi: Bir Puzzle Oluşturmak

Öz

Giriş: Hemşirelerin birçok kavramı, içinde barındıran bütüncül hemşirelik bakımın ne olduğunu analiz edebilmeleri için, bütüncül bakım sürecini, bileşenlerini ve etkilerini anlamlandırmaları gerekir. Bu bağlamda hemşirelerin bütüncül bakımı nasıl algıladıklarını belirlemek son derece önemlidir. Amaç: Araştırma; hemşirelerin bütüncül hemşirelik bakımına yükledikleri anlamları metaforlar aracılığı ile ortaya çıkarmak amacı ile yapılmıştır. Yöntem: Araştırma; nitel araştırma desenlerinden fenomenolojik (olgu bilim) desen kullanılarak, Mart-Nisan 2021 tarihlerinde bir eğitim araştırma hastanesinde araştırmaya katılmayı kabul eden 227 hemşire ile gerçekleştirilmiştir. Veriler araştırmacılar tarafından oluşturulan 10 soruluk veri toplama formu ile elde edilmiş, hemşirelerin demografik özelliklerini belirlemek amacıyla 9 soru sorulmuş ve bütüncül bakım kavramına ilişkin metaforları belirlemek amacıyla; ‟bütüncül hemşirelik bakım ………. gibidir; çünkü …………” cümlesinin tamamlamaları istenmiştir. Verilerin analizi beş aşamalı içerik analizi ile değerlendirilmiştir. Bulgular: Çalışmaya katılmayı kabul eden 227 hemşireden formu eksiksiz dolduran ve metafor-açıklama arasında tutarsızlık olmayan 209 hemşirenin 106 metafor ürettiği görülmüştür. Oluşturdukları bu metaforlar 9 kategoride "güven verici, kapsayıcı, yardım, gereksinim/gereklilik, zorunlu görev, profesyonellik, ahlaki sorumluluk, önemsiz/değersizlik, yol gösterici/yansımacı" incelenmiştir. Hemşireler %89 oranında pozitif metafor üretmişlerdir. En çok ürettikleri metaforlar; anne (n = 22, %20.8),
puzzle/yapboz (n = 15, %14.2), gökkuşağı (n = 6, %5.7), güneş (n = 5, %4.7), aile (n = 5, %4.7)’ dir. Sonuç: Hemşirelerin bütüncül bakım algısıyla ilgili ağırlıklı olarak olumlu metaforlara sahip olduğu saptanmıştır. Metaforlara ait her bir kategori için olumlu algıların desteklenerek geliştirilmesi, olumsuz algıların değiştirilebilmesi için strateji planlaması yapılmalıdır.
