Engagement of Older Adults with AI-Supported Healthcare Systems and the Risks of Age-Based Exclusion
Öz
This article argues that older adults’ engagement with healthcare systems supported by artificial intelligence (AI) cannot be explained solely by access to technology or individual levels of digital literacy. The central claim is that when older adults are unable to benefit fully from these systems, the problem is often framed as a personal failure to adapt, whereas the deeper issue is a form of sociotechnical exclusion produced at the intersection of data representation, design norms, limited explainability, inadequate institutional support, and age-based bias. To explore this issue, the article draws on a composite case involving an older woman who struggles to use an AI-supported chronic disease monitoring application. It first examines the opportunities associated with artificial intelligence in the care of older adults and the conceptual tensions surrounding these technologies. It then presents the composite case and analyses it through the lenses of digital exclusion, digital ageism, participatory design, care ethics, and nursing practice. The article concludes that the success of artificial intelligence in healthcare should be assessed not only in terms of accuracy, speed, and efficiency, but also in terms of its capacity to involve older adults in care processes in ways that are understandable, safe, fair, and dignified.
