Letter to the Editor: Evaluation of Triglyceride–Glucose Index and Metabolic Risk Factors in the Etiology of Central Retinal Artery Occlusion Short Title: TyG Index and CRAO Etiology Dear Editor, I read with great interest the article by Gündoğan et al. entitled “Triglyceride Glucose Index and Central Retinal Artery Occlusion” (DOI: 10.54005/geneltip.1683730). The authors addressed an important topic by investigating the relationship between the Triglyceride–Glucose (TyG) index, a recognized surrogate marker of insulin resistance, and central retinal artery occlusion (CRAO). Exploring the role of metabolic biomarkers in ocular ischemic diseases is valuable for elucidating the link between ophthalmic, cerebrovascular, and systemic vascular disorders. However, CRAO represents a heterogeneous clinical entity that cannot be explained by a single mechanism. According to the literature, most cases are thromboembolic or atherothrombotic in origin, while a smaller proportion results from vasospastic, inflammatory, cardioembolic, infectious, or coagulopathic causes (Hayreh, 2011; Varma et al., 2013). Therefore, the clinical significance of metabolic indices such as TyG may vary depending on the underlying pathophysiological mechanism of CRAO. Although Gündoğan et al. discussed the multifactorial pathogenesis of CRAO in their paper, the study did not classify patients into atherosclerotic, thromboembolic, or inflammatory subgroups, which limits the ability to interpret the TyG index in specific pathophysiological contexts. For instance, elevated TyG values might be expected in atherosclerosis-related CRAO, whereas the association could be weaker in inflammatory, vasospastic, or hematologic cases. Consequently, analyzing all cases within a single group may have diluted a potential relationship between TyG and CRAO. Future studies should employ multidisciplinary, clinical, and imaging-based etiological classifications for CRAO patients, including carotid artery evaluation, echocardiography, coagulation profile, rhythmological assessment, and autoimmune markers. Such an approach could better delineate the role of the TyG index in distinguishing atherosclerotic from non-atherosclerotic mechanisms. Another noteworthy point concerns the abbreviation “TGI” used in the study. In nearly all international publications, this index is universally referred to as the TyG (Triglyceride–Glucose) index, while the term “TGI” is rarely used. This inconsistency may reduce the recognizability and comparability of the findings within the broader literature. Adopting the internationally accepted abbreviation “TyG” in future publications would enhance the scientific visibility and citation potential of such studies. In conclusion, the study by Gündoğan et al. represents a valuable first step toward understanding the potential link between the TyG index and ocular ischemia. Nevertheless, in a model that does not account for the diverse etiologic subtypes of CRAO, the clinical interpretability of TyG remains limited. Further prospective studies comparing TyG across atherosclerotic and non-atherosclerotic CRAO subtypes will provide meaningful contributions to the literature. Conflict of Interest: None declared.
