İleri
glikasyon son ürünleri (AGE) karbonhidratların karbonil grubu ve amino
asitlerin serbest amino gruplarının bir araya gelmesi ile maillard reaksiyonu
sonucunda oluşan ürünlerdir. AGE oluşumu oldukça karmaşık bir süreçtir ve bu
süreçte sadece karbonhidratlar ve amino asitler yer almamaktadır. Oksidatif
stres ve karbonil stres gibi birçok faktör AGE oluşumunu tetiklemektedir. AGE
oluşum süreci yavaş ilerler. AGE oluşum sürecinde dokuların protein yapısına
zarar verir.
AGE metabolizmasında böbreklerin yeri oldukça önemlidir. AGE’lerin
serum seviyesini sabit tutulması ve vücuttan uzaklaştırılmasında böbrekler görev
almaktadır. Böbrekte kaynaklanan herhangi bir bozukluk veya herhangi bir hasar
AGE seviyelerinde artışa neden olmakta ve serum AGE seviyesindeki artışa bağlı
olarak üremik komplikasyonlar görülebilmektedir. İlerleyen böbrek yetmezliğinde
AGE seviyesi gittikçe artmakta ve böbrek fonksiyonlarının daha da azalmasına
neden olarak kısır bir döngü oluşmaktadır. Diyaliz ve böbrek transplantasyonu
ise serum AGE seviyelerini azaltan tedavilerdir.
