Effects of Epilepsy on Physical, Cognitive, Visual, Hearing, Speech, Communication and Nutritional Functions in Children with Cerebral Palsy

Öz

Epilepsy is a common symptom in children with cerebral palsy (CP). This study aimed to investigate the effects of epilepsy on physical, cognitive, visual, hearing, speech, communication and eating-drinking functions of children with CP. The study consisted of 297 children with CP. Data on epilepsy and functional status were retrospectively scanned through the records based on the Surveillance of Cerebral Palsy in Europe (SCPE) registration system. Physical, speech, communication and eating-drinking functional levels were classified according to the Gross Motor Function Classification System, Manual Ability Classification System, Communication Function Classification System, Viking Speech Scale and Eating-Drinking Ability Classification System. The effects of epilepsy on functions were examined using Chi Square test and Cramer’s V. Epilepsy was associated with ambulation (p<0.001), manual (p<0.001), cognitive (p<0.001), visual (p=0.038), communication (p<0.001) and eating-drinking (p<0.001) functions in children with CP with small to medium effect sizes. While epilepsy is known to strongly affect cognitive functions of children with CP, it can have negative effects on physical, visual, communication and eating-drinking functions. The diagnosis of epilepsy is included in standard registration systems; the use of these systems for national records allows multidisciplinary evaluation in the clinic and can be used reliably for research purposes.

Öz

Epilepsi, serebral palsili (SP) çocuklarda yaygın görülen bir semptomdur. Bu çalışmanın amacı epilepsinin SP’li çocukların fiziksel, bilişsel, görme, işitme, konuşma, iletişim ve beslenme fonksiyonlarına etkisini incelemekti. Çalışmaya 297 SP’li çocuk dahil edildi. Avrupa Serebral Palsi Surveyans (SCPE) sistemi değerlendirme formu ile kaydedilen çocukların epilepsi ve fonksiyonel durumları retrospektif olarak tarandı. Fiziksel, iletişim, konuşma ve beslenme fonksiyonları Kaba Motor Fonksiyon Sınıflandırma Sistemi, El Becerileri Sınıflandırma Sistemi, İletişim Fonksiyonları Sınıflandırma Sistemi, Viking Konuşma Ölçeği ve Yeme-içme Becerisi Sınıflandırma Sistemine göre belirlendi. Epilepsinin fonksiyonlar üzerine etkisi Chi Square testi ve Cramer’s V ile incelendi. Epilepsi, SP’li çocuklarda kaba motor (p<0.001), el becerileri (p<0.001), bilişsel (p<0.001), görme (p=0.038), iletişim (p<0.001) ve beslenme (p<0.001) fonksiyonları ile küçük ile orta düzey etki büyüklüğünde ilişkiliydi. Epilepsinin SP’li çocukların işlevleri arasında en fazla bilişsel alana etkileri bilinmekle birlikte fiziksel, görme, iletişim ve beslenmeyi de olumsuz etkilediği görülmüştür. Sonuçların genel olarak literatürdeki çalışmalarla benzerlik göstermesi SCPE standart kayıt sistemindeki verilerin restrospektif araştırmalarda da yararlı ve güvenilir olduklarını, bu sistemde yer alan epilepsinin gerek klinik izlemde, gerekse de araştırmalarda göz önüne alınması gereken bir tanı olduğunu göstermiştir.
