Sarkoidoz: On Altı Yaşında Bir Vaka Sunumu

Öz

Sarkoidoz, etiyolojisi bilinmeyen kronik multisistematik granülomatoz bir hastalıktır. Granülomlar, enfeksiyoz ya da organik ajanlara bağlı bir etiyolojiyi düşündürmekle beraber, nedeni hâlâ belli olmayan bir hastalıktır. On altı yaşında kız hasta, 4 aydır devam eden öksürük, halsizlik, bacaklarda morluklar yakınması ile başvurdu. Hastanın muayenesinde bacaklarında gözlenen eritema nodozum benzeri mor renkli hafif ağrılı en büyüğü 5 cm olan lezyon- lar dışında patolojik bulgu yoktu. Laboratuvar bulguların- da sedim 60/1 saat, CRP 15,66 mg/L olarak bulundu. Anjiotensin dönüştürücü enzim düzeyi yüksek olarak bulun- du. Toraks HRCT’de multipl mediastinal lenfadenopatiler, sağ orta ve altta mikronodüler parankimal infiltrasyonlar saptandı. Bronkoskopide lenfadenopati dışında patolojik bulgu izlenmedi, biyopside granülomatoz lezyonlar görül- dü. Bu bulgularla sarkoidoz düşünülen hasta klinik bulgu- ların çok ağır olmaması üzerine tedavisiz izleme alındı. Sarkoidoz, çocukluk çağında ender görülen bir hastalıktır. Spontan düzelme görülebildiği gibi, kronikleşebilmektedir. Hastaların muayene ve solunum fonksiyon testleri ile takibi gerekmektedir. Kliniğin ağırlığına göre steroid veya metot- rexat ile tedavi edilebilir, ancak sonuçlar tartışmalıdır. Ender görülen bir durum olduğu için bir vaka nedeniyle sarkoidozu vurgulamak ve literatürü derlemek istedik

Sarcoidosis: A 16 Year-Old Case Report

Öz

Sarcoidosis is a multisystemic, granulomatous disease with an unknown etiology. Although granulomas suggest an eti- ology related to infectious, and organic agents, their etiolo- gies are still unknown. A 16 year-old girl referred with complaints of cough, malaise, and painful lesions on legs lasting for 4 months. On physical examination, no patholo- gical sign was observed, except an erythema nodosum-like purple lesion, the largest one being 5 cm in diameter was detected on the tibial region. Erythrocyte sedimentation rate was 60/1 hour and CRP 15.66 mg/L. Angiotension conver- ting enzyme level was elevated. High resolution thoracal CT results were reported as multiple mediastinal lymphadeno- paties and micronodular parenchymal infiltrations at right middle and lower lobes. In biopsy, taken during bronchos- copy, granulamatous lesions were observed. Patient was suggestively diagnosed as sarcoidosis with these findings and followed up without treatment because of relatively lesser severity of these findings. Sarcoidosis is a rare disea- se of childhood, and can spontaneously resolve or become chronic. Follow-up of the patients with physical examinati- on and respiratory function tests are necessary. Steroids or methotrexate can be used for the treatment according to the clinical severity of the disease, but results are controversial. Since it a rarely seen condition, we want to emphasize the importance of sarcoidosis and review the literature
